Касае се за пациентка, при която е проведен МРТ на корем по повод хроничен панкреатит. В скенирания обем, посочена като странична находка, лезия в основата на десен бял дроб. Проведено КТ - в десен среден лоб мекотъканна спикуловидна формация с размери 24/20мм. Насочена за диагностично уточнение.
От проведената ПЕТ/КТ на цяло тяло с 199,3MBq 18F-FDG и време на фиксация 61мин. се установи:
Не се скенират зони на патологична активност в мозъчни структури.
Не се скенират зони на патологична активност в областта на глава и шия.
Не се скенират зони на патологична фиксация в бял дроб, медиастинум, аксили.
Описаната като формация в десен среден лоб, в момента се скенира като субсолиден нодул с размер 4мм с
ниска активност и категорично бенигнен вид. Прилежащо линеарно уплътнение като при остатъчни промени след реариране на субсегментна ателектаза без повишена активност . Не се скенират нодуларни лезии с вид на малигнени в белодробния паренхим двустранно.
Физиологична фиксация в черен дроб, слезка, панкреас, НБЖ.
Не се скенират зони на патологична фиксация в абдомен и малък таз.
Не се скенират зони на патологична фиксация в кости до скенираните нива.
Подкожна лезия в дясна аксила с умерено висока активност и вид на възпалителна - за оценка на локалния статус.
Заключение: Не се скенират метаболитно активни зони с вид на първичен тумор или вторични лезии. Находката в белия дроб, предвид претърпяното почти пълно обратно развитие за кратък период от време, може да се определи като бенигнена, вероятно с възпалителна генеза. Остатъчните променила с много ниска активност и също имат бенигнен вид.
Translation - English Type of scan: Positron emission tomography and computed tomography (PET-CT) with/without X-ray dye Urografin, with/without diuretics and radioactive sugar
The patient has had an MRI scan of abdomen due to chronic pancreatitis. There is a lesion at the base of the right lung, indicated as an incidental finding, in the scanned volume. The CT showed a soft-tissue spiculoid with dimensions 24/20 mm in the right middle lobe. The patient has been referred for further diagnostics.
The PET/CT scan of the whole body with 199.3 Mbq 18F-FDG and fixation time of 61 min. showed:
no pathological areas in the brain structures;
no pathological areas in the head and neck area;
no pathological areas of tracer fixation in the lung, mediastinum, axillas.
The described formation in the right middle lobe is currently a 4 mm subsolid nodule with low activity and is definitely benign.
There is an adjacent linear consolidation as in residual changes after reaeration of the subsegmental atelectasis without increased activity. The scan shows no malignant nodular lesions in the lung parenchyma bilaterally.
There is a physiological fixation in the liver, spleen, pancreas, adrenal gland.
The scan shows no pathological areas of tracer fixation in the abdomen and lesser pelvis.
The scan shows no pathological areas of tracer fixation in the bones up to the scanned levels.
The subcutaneous lesion in the right axilla is with moderate activity and appears to be inflammatory – for local staging.
Conclusion: The scan does not show metabolically active areas appearing as a primary tumor or secondary lesions. The finding in the lung, given the almost complete reversal for a short period of time, can be determined as benign, probably of inflammatory origin. The residual changes are with very low activity and also appear to be benign.

Source text - English In the past few decades, large segments of the Hutongs, the historic central districts of Beijing whose delicate urban fabric consists of narrow streets and courtyard buildings, have been destroyed to make way for much taller buildings. However, more recently, a new trend has developed: new residential districts are being built to look like the ones that have been lost. The few remaining historic districts are being discovered by the wealthy, and, at the same time, are becoming a tourist attraction. MAD Architects prepared a study addressing the future of the Hutongs. Small-scale interventions in the form of metallic bubbles, implanted in the urban fabric, are intended to attract additional target groups and to reinvigorate the districts. The program for this first realization: a wine shop. The adjacent existing house was refurbished and serves – as does a new one-storey structure on the opposite end of the courtyard – as retail space. The bubble cuddles up to the new building; its mirrored surface reflects the older building and the lush foliage, and almost seems to melt into air. The bubble shelters the stair that leads to the roof terrace and the patron’s WC. The structural framework consists of steel hollow section; the skin is made of 2 mm thick sheet steel that was welded to the framework off site and chromed. In the final step, the skin was polished to create a unified, shiny surface. The large oval skylight gives the small space a lofty, weightless flair.

Source text - English The velodrome for the 2012 Olympic Games in London is an elegantly curved shell structure with seating for 6,000 spectators. The overall form is divided horizontally into two parts. Enclosed within an earth embankment, the plinth bears the two upward curving ends of the cycle-racing track and houses the lower stands plus all functional spaces. A glazed circulation zone separates this from the timber-clad shell at the top, the form of which was determined by the sight lines from the upper tiers of seating on the two long sides of the track. Here, a 22-metre-high, broadly canti-levered structure is generated. In contrast, the edge of the roof next to the curves of the track dips far down to a level just above the circulation zone. The roof, with its double reverse curves, is supported by a cable-net structure, the tensile forces from which are transmitted to steel trussed girders that follow the line of the outer edge. Hollow timber elements, laid out to a 3.60-metre grid and supported on every cable node, bear an aluminium standing-seam roofing system. Top lights minimize the need for electric lighting. Compared with a conventional trussed-girder system, the present structure, consisting of tension elements and a compression ring, helped save roughly 1,000 tonnes of steel. The architects show that a dynamic form and intelligent construction need not be mutually exclusive. The structure is a fitting analogy with the spokes and rim of a bicycle wheel.

Колодрумът за Олимпийските игри през 2012 г. в Лондон притежава изящно извита скелетна конструкция с места за 6 000 зрители. Цялостната ѝ форма е разделена хоризонтално на две части. Ограден със земен насип, плинтът носи тежестта на двата извиващи се края на колоездачната писта и помещава по-ниските трибуни, плюс всички функционални пространства. Остъклената зона за движение разделя тази част от облицованата с дървен материал скелетна конструкция на върха, чиято форма била определена чрез видимите линии от горните редици на двете дълги страни на пистата. Тук е изградена 22-метрова стабилна носеща конструкция. За разлика, върха на покрива до завоите на пистата се спуска доста надолу, до ниво малко над зоната за движение. Покривът, със своите двойно извити линии, се поддържа от телено-мрежеста структура, чиято издържливост на якост се предава на стоманени подпорни греди, които следват линията на външния ръб. Кухите дървени елементи, поставени на 3.6-метрова мрежа и поддържани при всяка телена свръзка, носят алуминиевата покривна система. Горното осветление минимизира нуждата от разход на електричество. Source text - English The Lincoln Center in Manhattan, New York, is one of the most important venues for the performing arts in the world. It forms a small homogeneous city within the city. In the course of large-scale extensions and refurbishment measures to these buildings, which date from the 1960s, the architects set the glazed Hypar Pavilion – containing a cinema foyer and restaurant – opposite the Juilliard School. With its two-storey form, the pavilion mediates between the level of 65th Street and the pedestrian realm one storey higher around the foyers of the Lincoln Center buildings. To avoid losing any valuable pedestrian area as a result of the new construction, the architects designed the roof as a grassed accessible space – rather like a gently sloping, landscaped grandstand. The geometry of the hyperbolic paraboloid structure allows public access to the roof on the diagonal and also lends curvature to the internal soffit. The homogeneous quality of the travertine pavings and cladding to the plaza is continued in the restaurant by carpeting of the same colour. At the same time, the curved mahogany soffit extends out externally to north and south as the underside of the broad roof projection. The dematerialized quality of the all-glass facade and the balustrades of glass and wire netting bring out the planted roof to the full as a thin, continuous strip, so that one has the impression of a large, accessible sculpture hovering over the edge of the Lincoln Center.

Линкълн център в Манхатън, Ню Йорк, е едно от най-важните места за представяне на сценично изкуство в света. То представлява едно малко еднородно градче в рамките на големия град. По време на широкомащабните пристройки и мерките за подновяване на тези сгради, които са стоени през 60-те години на миналия век, архитектите поставят остъкления „Хипар Павилиън“ (Hypar Pavilion), който включва фоайе на кино и ресторант, срещу училището Джулиард. Постройката на два етажа е разположена между нивото на 65-та улица и един етаж по-високо, на пешеходната зона около фоайетата на сградите в Линкълн център. За да се избегне премахването на ценна пешеходна площ поради новото строителство, архитектите проектирали покрива като достъпно озеленено пространство – като леко наклонена, озеленена трибуна. Source text - English Six mysterious wooden cocoons stand in the snow like fine origami folded structures outside the Canadian city of Winnipeg, near the confluence of the Red and Assiniboine Rivers. As soon as the rivers freeze, kilometre long tracks and other areas are formed, which residents use for ice skating, ice hockey and walking. In recent times, the public has also been able to rest beneath temporary roofs that provide protection against the wind and weather. Wedge-shaped edge strips at the “base”, “ridge” and “spine” help to stabilize the sheets, for which the architects developed an optimum bending curvature. Cut-outs, holes and openings help to reduce excessive tensions in the wafer-thin material.

Шест загадъчни дървени постройки, приличащи на пашкули, стоят в снега като изящни оригами извън пределите на канадския град Уинипег, близо до мястото където Ред Ривър се влива в река Асинибойн.
