Nous n’avons pas pu mettre en évidence une attitude nationale de la prise en charge des PPROM entre 34 et 37 SA. Si l’on se base sur la literature, les benefices d’une expectative sont faibles et il semble plus sécure de proposer une provocation après 48 h de rupture des membranes en absence de signes infectieux ou de Streptococcus B-hémolitique. L’absence de consensus sur ce sujet mériterait une étude randomisée. Sur le plan national, un guideline pourrait être une aide à la pratique courante en attendant de savoir dépister une chorioamninite paucisymptomatique.
Translation - Arabic We couldn’t identify a national attitude for PPROM medical care between 34 and 37 weeks. If we rely on the medical literature, the benefits of expectative management are poor and it seems to be more secure to suggest an induction 48 hours after membrane rupture in the absence of signs of infection or of ß-hemolytic Streptococcus. Since consensus has not been reached, this topic could use a randomized study. Nationally, a guideline could be established to help the current practice while waiting for a better way to detect paucisymptomatic chorioamninitis.
