Mecanismo de acción de las células madre adultas
A pesar de todos los avances conseguidos hasta el momento en el campo de la medicina regenerativa, la aplicación clínica de las células madre adultas, principalmente de las derivadas de la médula ósea, continúa siendo el tema central y más debatido.
Aunque con su uso ya se han obtenido resultados positivos en varias enfermedades,3,16,22 todavía no se conocen bien los mecanismos mediante los cuales las células trasplantadas podrían mejorar o promover la regeneración de los tejidos. Para tratar de explicar estos mecanismos, se han sugerido varias hipótesis basadas en evidencias existentes, que incluyen la transdiferenciación celular, la fusión de células y un efecto autocrino/paracrino secundario a la liberación por las células de diferentes moléculas solubles o citocinas con acciones específicas, que incluyen varios factores de crecimiento. Probablemente se ejecute más de uno de estos mecanismos.28,41-43
Se ha comprobado que las células madre hematopoyéticas pueden producir varios elementos solubles que son esenciales para su acción y que incluyen factores que intervienen en la citoprotección, proliferación, diferenciación y migración celular, angiogénesis, respuesta inflamatoria, asentamiento celular y quizás con otras funciones aún no conocidas.28
Se ha sugerido que las señales emitidas por medio de factores liberados por las células residentes, o bien debidas a los contactos que se producen entre las células residentes y las trasplantadas, son capaces de estimular a estas últimas para su transdiferenciación en el tipo de célula residente circundante, lo que permitiría su integración al "nicho" apropiado para su acción regenerativa.41 También se ha plan teado la posibilidad de que alguno de los estímulos recibidos en el microambiente en que se han colocado, induzca la fusión de las células implantadas con las del tejido en que se han asentado, creando nuevas células con características funcionales que les permiten participar en la regeneración mística.28,41
Por otro lado, se ha comentado que los resultados que se han comunicado en algunas investigaciones no pueden explicarse solamente por la transdiferenciación o fusión celular, si se toma en cuenta el bajo número de nuevas células generadas que se pudo comprobar en el tejido en que se hizo el implante celular. Para explicar este hecho, se ha emitido la hipótesis de que factores solubles liberados por las células implantadas pueden desempeñar una acción esencial en la regeneración de los tejidos mediante un mecanismo paracrino que actúa estimulando en el sitio afectado a las células normales residentes. Estos productos solubles pueden también actuar, mediante un proceso autocrino, sobre las propias células trasplantadas que los secretan, modulando su biología y favoreciendo su autorrenovación, proliferación y continuidad de sus funciones. Existen varias experiencias que dan un fuerte apoyo a la intervención de mecanismos paracrinos en la regeneración de tejidos. Estudios experimentales han mostrado que la inyección en un sitio lesionado de medio de cultivo acondicionado mediante el cultivo de células madre adultas, puede producir efectos beneficiosos comparables a cuando el tratamiento se hace solo con células madre.28 También los resultados que se han visto con el uso de plasma rico en plaquetas o lisado plaquetario apoyan el criterio de la importante función paracrina de los elementos solubles.25,26,44,45

Adult Stem Cells Action Mechanism
In spite of the many advances made in the field of regenerative medicine till this moment, the clinical application of adult stem cells, primarily derived from the bone marrow, continues to be the central and most debated topic.
Even though its use has caused positive results in various diseases, 3,16,22 the mechanisms through which the transplanted cells could improve or promote tissue regeneration are still not well known. In order to try to explain these mechanisms, various hypothesis have emerged bases on existing evidence, which include cellular transdifferentiation, cellular fusion and an autocrine/paracrine effect, secondary to the liberation through the cells of different soluble or cytosine molecules with specific actions, including several growth factors. It is likely that more than one of these mechanisms will be executed. 28, 41-43
It has been confirmed that hematopoietic stem cells can produce various soluble elements essential for their action and which include factors that intervene in cellular citoprotection, proliferation, differentiation and migration, angiogenesis, inflammatory response, cellular settlement, and perhaps other presently unknown functions.28
It has been suggested that the signs emitted through liberated factors in resident cells, or rather due to the contact produced among the resident and transplanted cells, are capable of stimulating the latter for their transdifferentiation into the type of surrounding resident cell, which would allow its integration to the appropriate "niche" for its regenerative action.41 Another option has been laid out considering the possibility of having one of the received stimulus in the micro-environment where they haven been placed induce the fusion of the cells implanted with those settled in the tissue, creating new cells with functional characteristics that allow them to participate in the mystical regeneration. 28, 24
On the other hand, it has been said that the results communicated in some of the investigations cannot be solely explained by the transidfferentiation or cellular fusion, if the low number of generated cells, which could be proven in the tissue where the cellular implant took place, is taken into account. In order to explain this fact, the hypothesis has been made that the soluble factors liberated by the implanted cells can carry out an essential action in tissue regeneration through a paracrine mechanism that stimulates the normal resident cells in the affected area. These soluble products may also, through an autocrine process, act on the very transplanted cells which secrete them, modulating their biology and favoring their autorenovation, proliferation and the continuity of their functions. There are various experiences that strongly support the intervention of paracrine mechanisms in the regeneration of tissue. Experimental studies have shown that the injection of a conditioned cultivated medium through the cultivation of adult stem cells in a injured area can produce beneficial effects comparable to when the treatment is only done with mother cells.28 Also, the results seen with the use of plasma, rich in platelets or platelet lysate, support the criteria of the important paracrine function of the soluble elements. 25,26,44,45

So some kid accidentally dropped a dozen eggs all over the floor this morning.
You can just close your lovely eyes and imagine how extraordinarily well that ended.
But one particularly helpful/lazy child (bless him) did up and volunteer himself to make scrambled eggs.
Because, as he elucidated us all, “It won’t take much now — we’re already half way there. And if you don’t mind shells, crumbs and dust bunnies in your eggs — okay, yeah, make that a scrambled crunchy omelet.”
We all passed.
The dog didn’t.
The thermometer read a balmy -7F when our knees creaked out of bed and yessirree, eyeballs froze with the cold on the way out to the barn this morning.
Yeah, you know —
It was so cold that the dog almost opted to use the back restroom instead of relieving himself out there in the polar vortex that could suck any unsuspecting canine right into the land of penguins and polar bears.
I slipped on the ice on the back step sometime around sunrise.
So my posterior may or may not now be a precise purple hue and yes’m, we’re thinking all and all, it’s been a pretty mighty fine, memorable start to the day — because honest? And because the way you live your ordinary days is what adds up to your one extraordinary life, I may or may not have went and stuck a sticky note on that smudged and splattered mirror in the back mudroom, so anyone headed out (and down the deathtrap of those icy back steps) might catch a glimpse of themselves and ask that one question:
“If the Real You bumped into a Mirror of You today — would Real You like Mirror You?”
“You know,” — I ask the kid scrambling up the last dozen (undropped-still-intact-until-we-intentionally-decide-to-crack-them) eggs —
“so, if you bumped into an exact soul, body, mind, heart clone of you — would Real You like Mirror You?”
Because, sweetheart, you can go ahead and think bacon and eggs every day are as predictable as the sun rising willing in the east, but the most predictable thing about a day is that:
Somebody’s going to be late on you today, somebody’s going to interrupt you, somebody’s going to lose something on you, somebody’s going to say the wrong thing to you, somebody’s going to let you down, —
and every kid’s got to ask, every mom’s got to know, every woman’s got to decide, and every man’s got to determine:

Because the thing is?
The Real You meets Mirror You every morning first thing. So what if you decided first thing to be kind — the kind of person you’d love to face?
Those words right there, stuck right there on the end of my tongue —
and that sticky note with a heart and that string of questions now on every mirror in the house —
light in a reflection of morning glory…
Like His face smiling on all of ours in this mess of cracked egg shells that somehow isn’t quite as broken as before.

Pero un niño particularmente servicial y flojo a la vez (Dios lo bendiga) hizo su parte y ofreció hacer huevos revueltos.
Porque, como nos iluminó a todos: "No falta mucho que hacer ahora. Ya estamos a medio camino. Y si no te importa tener pedazos de cascarón, migajas y pelusas en tus huevos, entonces sí, que sea un omelette revuelto y crujiente."

Pero el perro no lo hizo.
El termómetro indicaba unos agradables -21°C cuando nuestras rodillas rechinaban al salir de la cama y sí señor, nuestros globos oculares se congelaron con el frío al caminar hacia el granero esta mañana.
Sí, tú sabes...
Hacía tanto frío que el perro casi optó por usar el baño de atrás en lugar de hacerse allá afuera en el vórtex polar que podría succionar a cualquier canino insospechado a la tierra de los pingüinos y los osos polares.
Me resbalé en el hielo en el escalón de atrás en algún momento del amanecer.
Así que mi posterior puede o no estar morado ahorita y sí, estamos pensando que con todo ha sido un grandioso buen y memorable comienzo del día. Porque honestamente si puedes respirar y murmullar tus gracias, sigue siendo un grandioso buen día.
Y porque la forma en la que vives tus días ordinarios es lo que compone tu vida extraordinaria. Pude haber ido o no a la parte de atrás y puesto una nota en ese espejo manchado para que cualquier persona a punto de salir (y bajar en la trampa mortal que son los escalones congelados) pueda tener un vistazo de sí mismos y preguntarse:
"Si tu verdadero Yo se encontrara con un reflejo de ti hoy, ¿le gustaría a tu verdadero Yo el reflejo de ti?"
"¿Sabes?", le pregunté al niño que revolvía la última docena de huevos (que no se han caído, que siguen intactos y que intencionalmente romperemos),"Si te encontraras con tu clon idéntico en alma, cuerpo, mente y corazón, a tu verdadero yo le agradaría tu reflejo?"
Porque, cariño, puedes pensar que comer huevos y tocino cada día es tan predecible como la dispuesta salida del sol en el oriente, pero lo que es más predecible acerca de un día es que:
Alguien te dejará esperando, alguien te va a interrumpir, a alguien se le va a perder algo tuyo, alguien te va a decir algo indebido, alguien te va a decepcionar.
Y cada niño debe preguntarse, cada mamá debe saber, cada mujer debe decidir y cada hombre tiene que resolver:
Si tu verdadero yo se encontrara con tu reflejo hoy, ¿le gustaría a tu verdadero yo tu reflejo? ¿Te sentirías comprendido o devaulado? ¿Te sentirías especial o especialmente criticado? ¿Te sentirías profundamente escuchado o ligeramente ofendido?
¿Será que tu reflejo practicaría su fe y el ministerio de la presencia y te daría el regalo de escuchar realmente? ¿O practicaría su propia importancia y toda esa moda de estar estresado y te ataría un nudo de anxiedad?
¿Tu presencia sería un presente o un pena?
¿A tu verdadero yo le gustaría pasar más tiempo con tu reflejo o menos?
¿A tu verdadero Yo le gustaría ser como tu reflejo o no?
Porque lo que pasa es que tu verdadero yo se encuentra con tu reflejo cada mañana a primera hora. Así que, ¿qué pasaría si decidieras a primera hora que serías amable, el tipo de persona con quien te encantaría encontrarte?
Esas palabras ahí, atoradas en la punta de mi lengua y esta notita en la que dibujé un corazón y escribí ese hilo de preguntas que ahora están en cada espejo de la casa, luz en el reflejo de gloria matutina;
Como Su rostro que sonríe sobre todos nostros en este desastre de cascarones de huevos rotos que de alguna manera no están tan quebrados como antes.
