The loss of a beloved child has hit this family hard. But ordinarily, the wider world would pay little attention to a child's death from infectious disease in this remote corner of Vietnam's Mekong Delta. Old scourges like dengue fever and typhoid still take a toll here, and HIV/AIDS is on the rise.
Yet Ngoan's death and more than 50 others in Southeast Asia over the past two years have raised alarms worldwide. Affected countries are struggling to take action; other nations are sending aid and advisers while stockpiling drugs and developing vaccines at home. And scientists have stepped up their research into the fateful traffic of disease between animals and people.
Why? Because Ngoan died of the flu.
To most of us, flu is a nuisance disease, an annual hassle endured along with taxes and dentists. Some people think a flu shot isn't worth the bother. But flu is easy to underestimate. The virus spreads so easily via tiny droplets that 30 million to 60 million Americans catch it each year. Some 36,000 die, mostly the elderly. It mutates so fast that no one ever becomes fully immune, and a new vaccine has to be made each year.
That's ordinary flu. But the disease that is taking lives in Southeast Asia is no ordinary flu. Its primary victims have been chickens, more than a hundred million of them, killed either by the virus or in often futile control efforts. It's not unusual for chickens to get flu; in fact, avian-flu viruses far outnumber human ones. But Robert Webster of St. Jude Children's Research Hospital in Memphis has studied flu viruses for 40 years and has never seen the likes of the one that killed Ngoan.
"This virus right from scratch is probably the worst influenza virus, in terms of being highly pathogenic, that I've ever seen or worked with," Webster says. Not only is it frighteningly lethal to chickens, which can die within hours of exposure, swollen and hemorrhaging, but it kills mammals from lab mice to tigers with similar efficiency. Here and there people have come down with it too, catching it from infected poultry like the chickens that died on Ngoan's farm a few days before she fell ill. Half the known cases have died.
In those deaths many public health experts hear the distant rumblings of a catastrophe. So far this virus—classified as H5N1 for two proteins that stud its surface like spikes on a mace—isn't good at passing from birds to people, let alone from one person to the next. "It can make that first step across, but then it doesn't spread easily from human to human," says Webster. "Thank God. Or else we'd be in big trouble."
Maybe H5N1 will never learn the trick of racing from person to person like the milder flus that empty offices and classrooms each year. Maybe it simply can't. Or maybe efforts to eradicate the virus—largely fitful and underfunded so far—will succeed. But experts are urging the world to prepare for the worst.
What is known about flu viruses' remarkable capacity to change and jump species has led to a sense of inevitability, a conviction that even if this menacing animal flu doesn't explode into a global pandemic that kills millions, another one will. "It's going to happen, at some point, that a virus like this changes to be able to transmit from one person to another," says Jeremy Farrar, an Oxford University doctor who works on the front lines of avian flu at the Hospital for Tropical Diseases in Vietnam's Ho Chi Minh City. "It's bound to happen. And when it does, the world is going to face a truly horrible pandemic."
After all, it has happened before.
In 1918, the final year of the savage trench fighting of World War I, something else began felling the soldiers. No one knows for sure when or where the Spanish flu emerged, though it certainly wasn't in Spain. As a neutral country, Spain had no wartime censorship, and the flu apparently got its false pedigree from news reports about outbreaks there in May 1918. In fact the disease was already spreading on both sides of the European front, laying low entire divisions through the spring and early summer. Then it seemed to subside.
In late summer, though, the Spanish flu returned, and this time its virulence was unmistakable. The sick took to their beds with fever, piercing headache, and joint pain. Many were young adults, exactly the group that normally shrugs off the flu. About five percent of the victims died, some in just two or three days, their faces turning a ghastly purple as they essentially suffocated to death. Doctors who opened the chests of the dead were horrified: The lungs, normally light and elastic, were as heavy as waterlogged sponges, clogged with bloody fluid.
After flashing through crowded military camps and troopships in Europe and the United States, the flu leaped out of uniform to ports and industrial cities. In Philadelphia, historian Alfred Crosby found, 12,000 people died of flu and pneumonia in October—759 in a single day. Schools and businesses were shut down and church services cancelled. Morgues overflowed.
By then the sickness had spread to the far corners of the planet, from the South Pacific to the Arctic. "Everybody on Earth breathed in the virus, and half of them got sick," says Jeffery Taubenberger of the Armed Forces Institute of Pathology in Maryland, who is trying to learn what made it such a killer. More than 50 million people died—at least three times as many as in the war. The best medical minds of the day could hardly believe that this was flu.
It was flu all right, but with a crucial difference that scientists are only beginning to understand. Scattered across Taubenberger's desk are translucent wax blocks the size of matchboxes. Borrowed from a pathology archive, they hold fingernail-size scraps of purplish tissue, sliced from the lungs of flu victims in U.S. military hospitals almost 90 years ago. In the mid-1990s Taubenberger and his colleagues realized that a sample from someone who died quickly, lungs still seething with virus, might still hold genetic traces of the killer. They were right: In 1996 lung tissue from a soldier who died in September 1918 at Fort Jackson, South Carolina, yielded pieces of the virus's genes.
The pickings soon got better. Inspired by Taubenberger's discovery, a retired pathologist named Johan Hultin traveled to a remote Alaska village and excavated a mass grave that had been hacked into the permafrost after the Spanish flu swept through in November 1918. One female body still contained intact lung tissue, preserved by the cold and sheer luck. Bit by bit, Taubenberger's group teased out the entire genetic sequence of the virus. They plan to finish publishing it this year.
So far this genetic blueprint hasn't revealed exactly what made the Spanish flu so deadly. No single gene or protein is the obvious culprit. But comparing the 1918 sequence to those of the flu viruses that wreak mild havoc each winter has confirmed what was long suspected: The Spanish flu virus had recently crossed into people from some unknown animal, leaving victims with little immunity to this new threat.
One reason you generally get over the flu after a few days' discomfort is that your immune system has seen it before and knows how to respond. This year's bug won't be a carbon copy of last year's, because the virus mutates constantly. But it will look similar enough that your body can almost always keep it in check.
Every so often, though, something new comes along from the animal world—a vast preserve of type A flu viruses, the ones that cause the most serious illness in humans. In far-flung studies in the late 1960s and 1970s, from Australia's Great Barrier Reef to lakes in northern Canada, Robert Webster and his colleagues tracked flu to its source. "Where do flu viruses come from?" he asks. "From the wild birds of the world, the wild aquatic birds—the waterfowl, the ducks, the shorebirds."
Dozens of flu subtypes inhabit the birds' guts, mostly harmless to their hosts or to any other creature. But occasionally one infects domestic poultry. Even more rarely, a bird virus or some of its genes slip into the much smaller pool of type A viruses that infect humans.
Normally a flu virus good at infecting birds can't attack humans because it isn't equipped to invade and grow in human cells. Until recently scientists thought avian viruses could gain that ability only by indulging in the viral equivalent of sex. Because flu viruses carry their genetic information on eight separate RNA segments, it's easy for different subtypes to swap genes if they happen to meet. The result: offspring with new abilities.
For an avian flu and a human flu to mix it up, they have to infect the same animal. Scientists have long considered the pig a likely mixing vessel, because pig cells have surface molecules that allow entry to both kinds of virus. A pig could conceivably catch a human flu from a farmer and a bird virus from, say, ducks at the same farm. The two viruses could then "reassort," creating a hybrid that—in the worst case—would now be able to infect human cells while still carrying bird-virus genes that would make it radically new to the immune system of the people who catch it, and unusually virulent.
Reassortment explains the two lesser flu pandemics of the 20th century, in 1957 and 1968. In each year a new flu subtype appeared, combining genes from the human virus that had been causing mild outbreaks in prior years with new genes from a bird virus. The new pandemic viruses raced around the world, together killing about two million people.
But in 1918, Taubenberger now believes, something different happened. "We think it's pretty likely that the virus was not derived from a previously circulating human virus," he says. All of its genes mark it as an animal virus, pure and simple, that somehow crossed to people without the help of genes from a previous human strain.
Now H5N1 is doing the same thing. So far, its steps across the species barrier are tentative, which is why it has caused tens of deaths, not millions. But as in 1918, doctors who have seen its effects close up are shaken.
The X-rays tell the story as Tran Tinh Hien, a doctor at the Hospital for Tropical Diseases in Ho Chi Minh City, clips them to a light box. In the first image, made the day the 18-year-old girl was admitted with bird flu, a whitish cloud appears at the base of her rib cage. Her lungs were partly filled with fluid. In a second film, four days later, the haze has spread throughout her chest. "All the lung tissue was destroyed," Hien says. "The process still happened when we treated." A week later the girl was dead.
So it went for Hien and his staff throughout January, when the latest bird-flu outbreak reached its peak in southern Vietnam. They cleared a 50-bed ward normally reserved for malaria and dengue fever and turned it into an isolation unit. They sustained patients with oxygen masks and ventilators and treated them with oseltamivir, or Tamiflu, an expensive antiviral drug that can fight H5N1. Nurses worked 24-hour shifts, gowned, masked, and goggled for protection against the virus. As Hien says: "We were pushed to the wall."
He and his staff did everything they could for their nine bird-flu patients. "Unfortunately," he says, "we could not save any lives."
As an elite facility, the Hospital for Tropical Diseases saw the bleakest face of the disease. Only the sickest patients were sent there, and by then they may have been beyond help. In fact H5N1 doesn't always kill. Some infections may even be so mild that they go unnoticed. But every hospital that has treated people seriously ill with avian flu has recorded shocking death rates.
It has been that way since 1997, when an H5N1 virus strain—a cousin of the one now plaguing Asia—first jumped to humans. Early that year an outbreak of the virus killed chickens in Hong Kong's rural New Territories. At that point, no one thought bird viruses threatened people directly. But this one broke the rules.
In May 1997 a three-year-old boy was admitted to a Hong Kong hospital with a cough and fever. His symptoms worsened rapidly and he had trouble breathing. He was given a flood of antibiotics and put on a ventilator, but within six days he was dead. Flu experts were astonished when secretions from the boy's windpipe yielded an H5N1 virus. It turned out to be the same one that had killed the chickens.
Still, his death looked like it might be a fluke until late in the year, when another 17 people checked into hospitals around Hong Kong with similar symptoms, and tests confirmed infection with H5N1. Five died. Many of the victims had visited one of the island's live-poultry markets.
Public health experts converged on Hong Kong, fearing that a 1918-style pandemic was about to explode. They persuaded the Hong Kong government to kill every last bird—1.5 million of them—in the farms and markets. The mass slaughter worked. That particular H5N1 virus was never seen again, and a public health disaster had been averted.
But in 2001 another deadly strain of H5N1 cropped up in Hong Kong's markets, and the city again began killing poultry. This time the respite was shorter, and by the beginning of 2002 chickens were again dying of flu. The fact was, the drastic measures in Hong Kong had left the source of all these viruses untouched. They were coming from outside Hong Kong—just across the territorial border in southern China.
China's Guangdong Province teems with hundreds of millions of chickens, ducks, and geese, many wandering freely through gardens, farms, and ponds. Flu viruses that rain into this sea of poultry in wild-bird droppings can spread and swap genes with abandon. The result: new strains not found in the wild. Among them was the H5N1 virus that gave rise to the bird flu now plaguing Asia.
Year after year it swapped genes with other avian-flu viruses, generating a plethora of new H5N1 variants. Year after year they besieged Hong Kong, which imports poultry from the mainland. By the end of 2003, they were infecting and killing birds across half of Asia.
Radiating from China, H5N1 strains reached South Korea and Japan in the north; they swept through Southeast Asia as far as Indonesia. Some experts and officials have suggested that the viruses traveled in the guts of wild waterbirds—geese, ducks, herons—which might have picked up the infection from farms. Governments embarrassed by their failure to halt the flu's spread welcome that idea. "They get a free lunch," says Yi Guan, a virologist at the University of Hong Kong. "Each time there's an outbreak, they say, 'It's migratory birds. I cannot control them. I cannot lock my sky!’ "
This summer the virus killed thousands of wild geese and gulls at a nature reserve in western China. It was the largest known outbreak in wild birds and a warning that in the future they might spread the disease far and wide. But Guan isn't ready to blame migratory birds for the spread so far. He thinks the virus has killed infected birds too quickly for them to fly long distances. Instead H5N1 probably hitchhiked across Asia in shipments of live poultry, in a disaster of our own making.
As it advanced, the virus began killing people again—by August the count stood at 40 in Vietnam, 12 in Thailand, 4 in Cambodia, and at least 1 in Indonesia. And it inflicted economic losses that, by one estimate, amounted to more than ten billion dollars in 2004 alone. Exports from Thailand's industrial chicken farms collapsed when the world learned of the outbreak there. In areas of Indonesia hit hard by the virus, more than 20 percent of the workers on commercial poultry operations lost their jobs.
Small farmers are suffering too. As one animal health official in Vietnam explains, "The birds are big treasures for the farmers." They scavenge for themselves, costing next to nothing to raise and putting good meat on the table. Vietnamese farmers lost some 40 million of these treasures in 2004, dead of flu or killed in control efforts. Even farms untouched by the disease were hit as fearful shoppers began avoiding poultry.
It may take a long time for Vietnam to regain its taste for chicken. At a riverside restaurant in the southern city of Can Tho, six veterinary officials—men responsible for keeping the district's poultry healthy—order prawns and a fish stew for lunch. They admit it: Not one of them will touch chicken these days.
H5N1 is all the more frightening because so much is unknown, starting with how it kills people. In a chicken, the virus spreads everywhere—gut, lungs, brain, muscle. In humans, like the 1918 flu, it devastates the lungs first and foremost.
Researchers at the University of Hong Kong have found that a victim's own immune system may be part of the problem. It reacts to the virus with a flood of chemical messengers that draw white blood cells into the lungs, where they trigger a massive inflammatory reaction. "It's kind of like inviting in trucks full of dynamite," says Malik Peiris, who led the work. Healthy tissue dies and blood vessels leak, filling the lungs with fluid.
But H5N1 may have more than one way to kill. This year researchers in Ho Chi Minh City, including Jeremy Farrar, detected H5N1 in a little boy who died in a coma, his brain inflamed but his lungs healthy until the very end. To Farrar it suggests that the virus can spread throughout the body. Others aren't sure.
It's one more bird-flu mystery.
Ask Keiji Fukuda and Tim Uyeki, flu epidemiologists at the Centers for Disease Control and Prevention (CDC) in Atlanta, what they would most like to know about the disease, and the questions come tumbling out. "How many people are infected? How many animals are infected?" asks Fukuda. Since 2004 only four countries have reported human illnesses. "Have there been no others?" Uyeki asks. "It's pretty likely there have been, but we have no information about that."
Then there's the mystery of exactly how people get infected. "Right now we believe that most cases are related to people somehow being exposed to sick or dying or dead poultry," says Fukuda. "Well, what does that mean?" he asks, ticking off possibilities. Does that mean people touched it? Did they eat it? Did they breathe in dust containing chicken feces? He's frustrated at the often sketchy reports from Asia. "What is really going on?" he asks. "This has very practical implications for fighting the disease."
The biggest question is whether the virus will start spreading like ordinary human flu. "Human to human—that's the one that we don't want to see," says Robert Webster. But already, H5N1 has given experts a few scares.
Nguyen Thanh Hung, a cement trader in Hanoi, says he feels well these days. He works, tends a small forest of potted plants, and spends hours a day jogging and doing breathing exercises. In his living room one day in February he shows off his hospital discharge papers as if they were his new lease on life. It's been just a month since Hung got over a serious bout of avian flu. "The doctor told me if I had been taken to the hospital one or two days later—no way," he says. His older brother, also infected, was not so lucky.
If Hung's survival is remarkable, so is the way he seems to have caught the disease. On a visit to their home village outside Hanoi he and his brother had shared a pudding made of raw duck blood, a Vietnamese delicacy. The duck must have been infected with H5N1, because Hung's brother came down with the flu a few days later. But Hung did not get sick for more than two weeks—not until the day after his brother died.
That's too late for the pudding to be the cause, Tim Uyeki believes. Hung probably got infected while caring for his brother in his final days.
H5N1 is also thought to have jumped from a girl dying of avian flu in Thailand to her mother and aunt, who nursed her. Several other cases of human-to-human transmission are suspected. But one step beyond the initial victim is all the virus seems to have managed. So far, sustained transmission—the chain-reaction spread of ordinary flu—is not in its repertoire.
It might gain that ability on its own, by mutating. Or it might swap genes with a human-flu virus. That could take place in an infected person who also catches ordinary flu, or in the classic flu mixing vessel, the pig. Still, no one knows whether the exchange would yield a nightmare virus or a dud.
Flu researchers at the CDC and in the Netherlands hope to find out in advance, by artificially breeding new viruses. In high-containment labs they are deliberately mixing and matching genes from H5N1 and human-flu viruses. Then they will test the hybrids to see whether any have inherited both the bird virus's virulence and the human virus's ability to spread. In effect, they are trying to create a pandemic flu strain in the lab.
Some critics think that's reckless, but Erich Hoffmann, a St. Jude researcher who helped develop a genetic engineering technique for flu, says the experiments are key to learning what we may eventually face. "It's basically the best simulation one could have in the lab of what possibly could happen in nature."
If we're lucky, he says, all the hybrid viruses will be harmless or won't thrive, suggesting that H5N1 may never spawn a pandemic. "That of course would be good news," says Hoffmann. And if the news is less reassuring? Then scientists monitoring H5N1 in Asia would have a clue about what to watch for—what genetic changes in the virus might signal big trouble ahead.
That knowledge might buy some warning time, but no one wants to sit and wait for H5N1 to make its move. And everyone agrees on the best way to head it off: Eradicate H5N1 in poultry, so people can't catch it. It's simple—but not easy.
Howard Wong, a senior veterinary officer in Hong Kong, knows what it takes. Since 2003, after multiple outbreaks and two wholesale poultry slaughters, the territory has kept its farms and markets free of H5N1. Wong is proud of his success. "We were very happy in 2004, when it was everywhere else, and we managed to hold it back."
Here are some steps Wong and other officials took: vaccinating every chicken in Hong Kong against H5N1; regularly testing chickens, pet birds, even wild birds; shutting down the hundreds of live-poultry stalls twice a month to disinfect them; and inspecting farms and markets obsessively. "We're almost at the limit of what we can do," says Wong. "It's like holding up this wall, and this wall just gets heavier all the time."
And as one aid official in Vietnam puts it, Hong Kong is rich, and an island. Vietnam is neither. In the winter of 2003-04, H5N1 broke out in most of the country's 64 provinces. To fight it, the country slaughtered tens of millions of chickens. The epidemic seemed to decline, and in March the government declared victory.
But in late 2004 avian flu roared back, infecting birds and people along the length of the country. Cooler weather, which favors the survival and spread of the virus, had apparently brought it out of hiding. By February of this year Anton Rychener, the representative of the United Nations Food and Agriculture Organization (FAO) in Hanoi, was beside himself. "Why the hell are we sitting here a year later with literally the same pattern of outbreak?" he asked a roomful of officials. A year of meetings and emergency initiatives had done little to change the conditions that made Vietnam ripe for viral spread.
In the countryside, chickens peck in yards and stalk through underbrush, mingling with birds from other farms. Poultry markets act as viral swap meets, and unsold birds are taken back home with any viruses they picked up. Farmers with sick birds have little incentive to speak up and see their flocks destroyed: The government can afford to pay less than half of a bird's market value as compensation. And then there are the ducks
The duck, says Webster, is "the Trojan horse of this outbreak." His group has found that unlike chickens, infected ducks often seem healthy, able to waddle, swim—and spread the virus in their droppings. In Vietnam and other parts of Southeast Asia, they spread it far and wide because duck herders drive their flocks from one rice field to the next, following the harvest, so the birds can fatten on leftover grains.
Inefficiency, tradition, and scarce funds have all slowed Vietnam's efforts to change such practices. And although officials gamely declare that they are making progress, last year's confidence has faded. When asked whether he expects his district to be hit by bird flu again next year, a veterinary officer in Can Tho says, "It's not my expectation, but I think it will happen."
Thailand has made more headway. Wealthier and more developed than Vietnam, it reimburses farmers more generously when birds are slaughtered. It tests ducks for H5N1 and allows only virus-free flocks to move around the countryside. And it has enlisted nearly a million village volunteers to watch for unusual chicken die-offs. By early this year it had driven the virus back into just a handful of provinces.
Even so, Wantanee Kalpravidh, the FAO's regional coordinator for avian influenza in Southeast Asia, says the country cannot rest easy. It has long borders with Laos and Cambodia, destitute lands struggling with their own avian-flu outbreaks. Thailand's efforts could quickly be undone if a crate or two of infected poultry slipped in. As Kalpravidh points out, "Birds need no visa or passport."
Nor do viruses. Suppose H5N1 lives up to the fears and picks up a new talent for contagion. Somewhere, probably deep in the countryside, a pandemic spark would ignite. A person sick with bird flu would infect his family, who would pass the disease to friends and neighbors. Exponential spread might follow.
Computer simulations suggest that public health officials just might be able to douse the spark by flooding the region with antiviral drugs, treating the hundreds of thousands of people who are at highest risk for infection. But the strategy could succeed only if the outbreak were detected within a few weeks and the virus spread slowly at first. And the infrastructure and skills required are lacking in much of Asia.
If containment failed, in a few more weeks the newborn pandemic would hit major cities. There it would infect people with passports and plane tickets. The rest of the world would be hours away.
Richer countries are scrambling to prepare. Because Tamiflu can protect against H5N1 as well as treat it, governments are building up stockpiles, and drugmaker Roche is hard-pressed to keep up with demand. The U.K. has ordered enough for 15 million people, a quarter of its population, and France almost as much. The U.S. has opted for a smaller stockpile—just 2.3 million treatments so far. It is also pinning its hopes on a vaccine.
Ordinary flu vaccines contain flu virus, grown in fertilized chicken eggs, then killed and split into pieces. But because it's so deadly, H5N1 requires expensive safety measures and tends to kill the eggs needed to grow it.
So Webster's group turned to genetic engineering, altering one gene to tame the virus and splicing in others to speed its growth. By August human tests of a vaccine made from the altered virus showed early signs of success. The U.S. government has already ordered two million doses.
Two million doses would be scant protection for a country of nearly 300 million. But officials hope the vaccine formula will be fully tested and ready before it is ever needed. Makers would know how to produce it and could boost production fast, says Anthony S. Fauci, director of the National Institute of Allergy and Infectious Diseases. At the first sign of a pandemic, "We'll be able to say: Go, take off the brakes and make millions and millions of doses."
But again comes the refrain, no one knows. No one knows whether an H5N1 pandemic strain—if it ever emerges—will be similar enough to the virus now stalking Asia for this vaccine to offer full protection. No one can even be sure H5N1 is the right threat to focus on. Several other bird-flu viruses have recently shown signs of infecting humans, though none has proved as deadly as H5N1.
Here is what we know for sure. One day a new flu pandemic will come, and one day it will pass. And then the killer strain, tamed by our immune systems and the passage of time, will fade into the background of nuisance flus.
It even happened to the worst of them all. Did you catch the flu last winter? There's a chance it was a direct descendant of the Spanish flu. If so, you were lucky. The heir to 1918 is one of the milder flu strains around today.

Algunas personas piensan que la vacuna antigripal no vale la pena. Pero la gripe es fácil de subestimar. El virus se propaga fácilmente por medio de pequeñas gotas, contagiando de 30 a 60 millones de estadounidenses anualmente. Aproximadamente 36,000 mueren, en su mayoría ancianos. Muta tan rápido que nadie nunca se vuelve completamente inmune, y una nueva vacuna debe ser desarrollada cada año.
Esa es la gripe ordinaria. Pero la enfermedad que está quitando vidas en el Sudeste de Asia, no es la gripe común. Las primeras víctimas han sido pollos, más de cien millones han muerto ya sea por el virus, o a menudo, en un esfuerzo casi inútil de controlarlo. No es inusual que los pollos se contagien de gripe, de hecho, virus de gripe aviar supera por mucho a los humanos. Robert
Webster del Hospital de investigación infantil San Judas en Memphis, ha estudiado el virus de la gripe por 40 años y nunca ha visto uno como el que mató a Ngoan.
Webster señala "El virus desde cero es probablemente el peor virus de influenza qué he visto o con el que he trabajado, en términos de ser altamente patógeno ". No solo es terriblemente letal para los pollos, que pueden morir hinchados y con hemorragia a las a pocas horas de ser expuestos. Mata a mamíferos con las misma eficacia, desde ratones de laboratorio hasta tigres. Por todas partes la gente lo contrae de aves infectadas, como los pollos que murieron en la granja de Ngoan pocos días antes de que ella enfermara. La mitad de los casos confirmados han muerto.
Sobre esas muertes, muchos expertos en salud pública escuchan los lejanos rumores de una catástrofe. Hasta ahora este virus (clasificado como H5N1 por dos proteínas que se fijan en la superficie como púas en un mazo) no es efectivo para pasar de las aves a las personas, y mucho menos de una persona a otra. Webster dice "Puede darse el primer paso, pero después no se propaga fácilmente entre humanos, Gracias a Dios. De lo contrario estaríamos en un grave problema.
Tal vez H5N1 nunca aprenderá el truco de cómo propagarse de una persona a otra, como la gripe más leve que vacía las oficinas y las aulas cada año. Quizá simplemente no pueda. O tal vez los esfuerzos para erradicar el virus (hasta ahora irregulares e insuficientes en gran medida) tendrán éxito. Los expertos instan al mundo a prepararse para lo peor.
Lo que sabemos de la capacidad de adaptación del virus de la gripe para cambiar y modificarse de un tipo a otro, ha llevado a pensar en lo inevitable, a la convicción de que incluso si esta gripe animal intimidantes no explota en una pandemia mundial que mate a millones, otra lo hará. "Va a suceder en algún momento que un virus como este se modifique para poder transmitirse de una persona a otra", dice Jeremy Farrar, un médico de la Universidad de Oxford que trabaja en la primera línea de batalla de la gripe aviar en el Hospital de Enfermedades Tropicales de la ciudad vietnamita de Ho Chi Minh. "Es inevitable que ocurra. Y cuando esto pase, el mundo se enfrentará a una pandemia realmente terrible".
Después de todo, ya ha sucedido antes.
En 1918, el último año de enfrentamientos violentos de trincheras de la Primera Guerra Mundial, algo distinto empezó a derribar a los soldados. Nadie sabe con certeza cuando o dónde surgió la gripe española, aunque ciertamente no fue en España. Como país neutral, España no tenía censura en tiempos bélicos, y la gripe aparentemente obtuvo su falso linaje basándose en los reportajes sobre los brotes que ocurrieron allí en mayo de 1918. De hecho, la enfermedad ya se estaba propagando en ambos bandos del frente europeo, manteniéndose a la baja ambas divisiones a lo largo de la primavera y principios del verano. Luego pareció disminuir.
Sin embargo, a finales del verano la gripe española volvió, y esta vez su mordacidad fue inequívoca. Los enfermos estuvieron en cama con fiebre, dolor de cabeza agudo y dolor articular. Muchos eran jóvenes adultos, exactamente el grupo que normalmente ignoran la gripe. Alrededor del cinco por ciento de las víctimas murieron, algunas en solo dos o tres días, sus rostros se volvían de un color morado horrendo mientras que, fundamentalmente se asfixiaban hasta morir. Los doctores que abrieron los pechos de los muertos estaban horrorizados: Los pulmones que son normalmente ligeros y elásticos, eran tan pesados como esponjas empapadas de agua, obstruidos por flujo sanguíneo.
Después de pasar por campamentos militares atestados y buques de guerra en Europa y los Estados Unidos, la gripe salto del uniforme a los puertos y ciudades industriales. En Filadelfia, el historiador Alfred Crosby descubrió que 12,000 personas murieron en un solo día de gripe y neumonía en octubre de —759. Escuelas y negocios se suspendieron y los servicios religiosos fueron cancelados. Las morgues se desbordaban.
Para entonces la enfermedad se había extendido a todos los rincones del planeta, desde el Pacífico Sur hasta el Ártico. "Todo el mundo en la Tierra respiró el virus, y por lo menos la mitad se enfermó", dice Jeffery Taubenberger del Instituto de Patología de las Fuerzas Armadas en Maryland, quien está tratando de descubrir qué fue lo que lo hizo tan letal. Más de 50 millones de personas murieron, tres veces más que en la guerra. Las mentes médicas más brillantes de la época difícilmente podían creer que se trataba de una gripe.
Fue una gripe, pero con una diferencia crucial que los científicos apenas están empezando a entender. Dispersos sobre el escritorio de Taubenberger hay bloques de cera translúcida del tamaño de una caja de fósforos, fueron extraídos de un archivo de patología y contienen trozos de tejido morado del tamaño de una uña, extraídos de los pulmones de las víctimas de la gripe en los hospitales militares de E.U. hace casi 90 años. A mediados de los 90, Taubenberger y sus colegas se dieron cuenta de que una muestra de alguien que murió repentinamente, con el virus rebosando en los pulmones, podría aún contener rastros genéticos del asesino. Tenían razón: en 1996 el tejido pulmonar de un soldado que murió en septiembre de 1918 en Fort Jackson, Carolina del Sur, produjo trozos de los genes del virus.
Las recolecciones pronto mejorando. Johan Hultin un patólogo jubilado, Inspirado por el descubrimiento de Taubenberger, viajó a un pueblo remoto de Alaska y excavó una fosa común que había sido penetrada en el permahielo después de que la gripe española arremetiera en noviembre de 1918. El cuerpo de una mujer todavía contenía tejido pulmonar intacto, preservado por el frío y la suerte. Poco a poco, el grupo de Taubenberger han estado descifrando toda la secuencia genética del virus. Planean terminar de publicarlo este año.
Hasta ahora este prototipo genético no ha revelado exactamente qué hizo que la gripe española fuera tan mortal. Ningún gen o proteína es el evidente culpable. La estructura del virus de 1918 comparado con el virus de la gripe que causan leves estragos cada invierno, se ha confirmado lo que se sospechaba desde hace tiempo: Recientemente el virus de la gripe española se había transmitido a una persona de algún animal desconocido, dejando a las víctimas con poca inmunidad ante esta nueva amenaza.
Una de las razones por las que generalmente se supera la gripe después de unos días de malestar, es que el sistema inmunológico la reconoce y cómo atacar. El virus de este año no será una copia exacta del año pasado, porque el virus muta constantemente. Pero será bastante similar como para que el cuerpo casi siempre pueda mantenerlo bajo control.
Sin embargo, cada cierto tiempo surge algo nuevo del mundo animal, una vasta reserva de los virus de gripe tipo A, los que causan las más graves enfermedades en los seres humanos. En extensos estudios realizados a finales de los años 60 y 70, desde la Gran Barrera de Coral de Australia hasta los lagos del norte de Canadá, Robert Webster y sus colegas rastrearon el origen de la gripe. "¿De dónde vienen los virus de la gripe?", se pregunta. "De las aves silvestres del mundo, las aves acuáticas, las anátidas, los patos, las aves costeras".
Decenas de subtipos de gripe habitan en las entrañas de las aves, en su mayoría son inofensivas para sus huéspedes o para cualquier otro animal. Ocasionalmente infectan a las aves de corral. Muy raras veces, un virus aviar o algunos de sus genes se introducen en una reserva más pequeña de los virus tipo A que infecta a los humanos.
Normalmente el virus de la gripe que contagia con más eficacia a las aves no puede atacar a los humanos, porque no está provisto para invadir y desarrollarse en las células humanas. Hasta hace poco, los científicos pensaban que el virus aviar podía adquirir esa capacidad solamente llevando a cabo el equivalente viral del sexo. Debido a que el virus de la gripe tiene la información genética en ocho segmentos separados de ARN, es fácil para los diferentes subtipos de genes intercambiase si se encuentran. El resultado: descendencia con nuevas habilidades.
Para que una gripe aviar y una gripe humana se combinen, tienen que infectar al mismo animal. Durante mucho tiempo los científicos han considerado que posiblemente el cerdo sea el recipiente donde se mezclan, porque las células de cerdo tienen moléculas en la superficie que permiten la entrada de ambos tipos de virus. Sería posible que un cerdo contraiga una gripe humana de un granjero y un virus aviar, digamos, de patos de la misma granja. Los dos virus podrían entonces agruparse, creando un híbrido que, en el peor de los casos, sería ahora capaz de infectar células humanas mientras sigue llevando genes de virus de ave que lo harían radicalmente nuevo para el sistema inmunológico de las personas que lo contraigan, e inusualmente potente.
Esta recombinación explica las dos pandemias menores de gripe del siglo XX, en 1957 y 1968. Cada año un nuevo subtipo de gripe aparecía, combinando genes del virus humano que había estado causando brotes leves en años previos con nuevos genes de un virus aviar. Los nuevos virus pandémicos se propagaron por todo el mundo, matando en conjunto a unos dos millones de personas.
En 1918, Taubenberger ahora cree que algo diferente ocurrió. "Creemos que es muy probable que el virus no proviene del tipo humano que circulaba con anterioridad ", dice. Todos sus genes lo clasifican como un virus animal, puro y simple que de alguna manera contamina a las personas sin la ayuda de los genes de una cepa anterior humana.
Ahora el H5N1 está haciendo lo mismo. Hasta ahora es incierto su avance a través de la barrera de especies, por lo que ha causado decenas de muertes, no millones. Pero al igual que en 1918, los médicos que han visto de cerca sus efectos están conmocionados.
Radiografías nos revelan la historia mientras Tran Tinh Hien, un médico del Hospital de Enfermedades Tropicales de la Ciudad de Ho Chi Minh, las sujeta a una caja de luminosa. En la primera placa, tomada el día que la chica de 18 años fue admitida con gripe aviar, una nube blanquecina aparece en la base de su caja torácica. Sus pulmones estaban parcialmente cubiertos de líquido. En una segunda radiografía, cuatro días después, la bruma se ha extendido por todo su pecho. "Todo el tejido pulmonar fue destruido", dice Hien. "El proceso todavía ocurrió mientras lo tratábamos". Una semana después, la chica había muerto.
Así fue todo el mes de enero para Hien y su personal, cuando el último brote de gripe aviar alcanzó su punto máximo en el sur de Vietnam. Despejaron un pabellón de 50 camas, normalmente destinado para la malaria y la fiebre del dengue, y lo convirtieron en una unidad de aislamiento. Mantuvieron a los pacientes con máscaras de oxígeno y ventiladores, y los trataron con oseltamivir o Tamiflu, un medicamento costoso antiviral que puede combatir el H5N1. Las enfermeras trabajaban en turnos de 24 horas, con batas, máscaras y gafas de protección contra el virus. Como dice Hien: "Nos pusieron contra la pared".
Él y su equipo hicieron todo lo que pudieron por sus nueve pacientes con gripe aviar. "Desafortunadamente", dice, "no pudimos salvar ninguna vida".
Como institución de élite, el Hospital de Enfermedades Tropicales vio el lado más sombrío de la enfermedad. Únicamente los pacientes más graves fueron enviados allí, y para entonces no había manera de ayudarlos. De hecho, el H5N1 no siempre es letal. Algunas infecciones pueden ser tan leves que pasan desapercibidas. Pero todos los hospitales que han atendido a personas gravemente enfermas de gripe aviar han registrado tasas de mortalidad alarmantes.
Así ha sido desde 1997, cuando una cepa del virus H5N1 (primo del que ahora asola Asia) saltó por primera vez a los humanos. A principios de ese año un brote del virus mató a los pollos en los Nuevos Territorios rurales de Hong Kong. En ese momento, nadie pensó que los virus aviares afectaran directamente a las personas. Pero este rompió las reglas.
En mayo de 1997 un niño de tres años fue ingresado en un hospital de Hong Kong con tos y fiebre. Tenía problemas para respirar y sus síntomas rápidamente empeoraron. Se le administró una vasta cantidad de antibióticos y le conectaron un respirador, pero a los seis días estaba muerto. Los especialistas se asombraron cuando las secreciones de la tráquea del niño produjeron el virus H5N1. Resultó ser el mismo que había matado a los pollos.
Aun así, su muerte parecía ser una casualidad hasta finales del año, cuando otras 17 personas ingresaron a hospitales de Hong Kong con síntomas similares y las pruebas dieron positivo a H5N1. Cinco murieron. Muchas de las víctimas habían visitado un mercado de aves domesticas de la isla.
Los expertos en salud pública se reunieron en Hong Kong, temiendo que una pandemia como la de 1918 estaba a punto de estallar. Convencieron al gobierno de Hong Kong para que sacrificara hasta la última ave (millón y medio) de las granjas y mercados. El asesinato masivo funcionó. Ese virus H5N1 en particular no se volvió a ver, y se evitó un desastre de salud pública.
Pero otra cepa mortal de H5N1 surgió en 2001 de los mercados de Hong Kong, y la ciudad empezó de nuevo a matar aves de corral. Esta vez la prorroga fue más corta, a principios de 2002 los pollos volvieron a morir de gripe. El hecho era que las medidas drásticas de Hong Kong habían dejado indemne el origen de todos estos virus. Venían del exterior de Hong Kong, justo al otro lado de la frontera en el sur de China.
En la provincia China de Guangdong proliferan cientos de millones de pollos, patos y gansos, muchos de ellos vagando libremente por jardines, granjas y estanques. Los virus de la gripe se precipitan en este mar de aves de corral, en los excrementos de las aves silvestres se pueden propagar e intercambiar genes con desenfreno. El resultado: nuevas cepas que no se encontraban en el medio silvestre. Entre ellas el virus H5N1 que dio lugar a la gripe aviar que ahora azota Asia.
Cada año el intercambió genético con otros virus de la gripe aviar va generando una plétora de nuevas variantes del H5N1. Año tras año asediaron Hong Kong, que importa aves de corral del continente. A finales de 2003, estaban infectando y matando aves por toda la mitad de Asia.
Emitidas desde China, las cepas de H5N1 llegaron a Corea del Sur y al norte de Japón; se extendieron por el sudeste asiático hasta Indonesia. Algunos expertos y funcionarios han sugerido que los virus viajaron en las entrañas de las aves acuáticas silvestres (gansos, patos, garzas), las cuales podrían haber contraído la infección en las granjas. Las autoridades avergonzadas por su fracaso en detener la propagación de la gripe acogen con beneplácito esa idea. "Tienen un almuerzo gratis", dice Yi Guan, virólogo de la Universidad de Hong Kong. "Cada vez que hay un brote, dicen, 'Son aves migratorias. No puedo controlarlas. ¡No puedo bloquear el cielo!' "
Este verano el virus mató a miles de gansos y gaviotas silvestres en una reserva natural en el oeste de China. Fue el mayor brote conocido en aves silvestres y una advertencia de que en el futuro podrían propagar la enfermedad a lo largo y ancho. Hasta ahora Guan no está dispuesto a culpar a las aves migratorias por la propagación. Cree que el virus ha matado a las aves infectadas demasiado rápido como para que puedan volar a grandes distancias. En cambio, el H5N1 viajó haciendo autostop a través de Asia en cargamentos de aves de corral vivas, en un desastre de nuestra propia cosecha.
A medida que avanzaba, el virus empezó a matar de nuevo a la gente —en agosto el recuento era de 40 en Vietnam, 12 en Tailandia, 4 en Camboya y al menos 1 en Indonesia—. Infligió pérdidas económicas que, según un estimado, ascendieron a más de diez billones de dólares solo en 2004. Las exportaciones de las granjas industriales de pollos de Tailandia colapsaron cuando el mundo se enteró del brote. En las zonas de Indonesia gravemente afectadas por el virus, más del 20 por ciento de los trabajadores de las actividades comerciales avícolas perdieron sus empleos.
Los pequeños agricultores también lo están padeciendo. Como explica un funcionario de sanidad animal de Vietnam, "las aves son un gran tesoro para los granjeros". Se alimentan por sí solas, no cuesta casi nada criarlas y ponen buena carne sobre la mesa. Los granjeros vietnamitas perdieron unos 40 millones de estas joyas en 2004, muertos por la gripe o por un esfuerzo en controlarla. Incluso las granjas indemnes de afección sufrieron el impacto cuando los clientes temerosos empezaron a evitar las aves de corral.
Puede llevar mucho tiempo para que Vietnam recobré su gusto por el pollo. En un restaurante a la ribera en la ciudad sureña de Can Tho, seis funcionarios veterinarios (hombres responsables de mantener saludables las aves de corral de la región), ordenaron gambas y un guiso de pescado para el almuerzo. Lo reconocen: Ninguno de ellos probará el pollo en estos días.
El H5N1 es aún más aterrador porque se desconocen muchas cosas, empezando por cómo mata a la gente. En un pollo, el virus se propaga por todas partes: intestinos, pulmones, cerebro, musculatura. En los seres humanos, como la gripe de 1918, principalmente devasta los pulmones.
Investigadores de la Universidad de Hong Kong han descubierto que el propio sistema inmunológico de la víctima puede ser parte del problema. Reacciona al virus con un flujo de mensajeros químicos que llevan glóbulos blancos a los pulmones, donde desencadenan una reacción inflamatoria masiva. "Es como hospedar a camiones repletos de dinamita", dice
Malik Peiris, que lidero el trabajo. El tejido sano muere y los vasos sanguíneos se filtran, llenando los pulmones de líquido.
H5N1 puede tener más de una manera de matar. Este año investigadores en la Ciudad Ho Chi Minh, incluyendo a Jeremy Farrar, detectaron H5N1 en un niño pequeño que murió en coma, con el cerebro inflamado pero sus pulmones sanos hasta el final. Para Farrar, esto indica que el virus puede propagarse por todo el cuerpo. Otros no están tan seguros.
Es un misterio más de la gripe aviar.
Pregunten a Keiji Fukuda y a Tim Uyeki, epidemiólogos de la gripe en los Centros para el Control y la Prevención de Enfermedades (CCPEEU) de Atlanta, qué es lo que más les gustaría saber sobré la enfermedad, y las preguntas se desploman. "¿Cuántas personas están infectadas? ¿Cuántos animales están infectados?" pregunta Fukuda. Desde 2004, solo cuatro países reportarían contagios en humanos. "¿No ha habido otros?" pregunta Uyeki. "Es muy probable que haya habido, pero no tenemos información al respecto."
El misterio está en cómo se infecta la gente exactamente. "Por ahora creemos que la mayoría de los casos están relacionados con personas que, de alguna manera fueron expuestas a aves de corral enfermas, moribundas o muertas", dice Fukuda. "Bueno ¿y qué significa esto?" pregunta, marcando las posibilidades. ¿Significa que la gente lo tocó? ¿Se lo comieron? ¿Respiraron el polvo que contenía heces de pollo? Frustrado por los reportes a menudo incompletos de Asia. "¿Qué está pasando realmente?", pregunta. "Esto tiene implicaciones prácticas para la lucha contra la enfermedad".
La pregunta más importante es si el virus comenzará a propagarse como la gripe humana común. "De humano a humano, eso es lo que no queremos ver", dice Robert Webster. Pero, el H5N1 ya ha dado a los expertos algunos sustos.
Nguyen Thanh Hung, un comerciante de cemento en Hanoi, dice que se siente bien en estos días. Trabaja cuidando un pequeño bosque de macetas y plantas, y pasa horas al día trotando y haciendo ejercicios de respiración. Un día de febrero, en su sala de estar, presume sus papeles de alta del hospital como si fueran una nueva oportunidad de vida. Ha pasado solamente un mes desde que Hung se recuperó de un ataque grave de gripe aviar. "El médico me dijo que si hubiera ido al hospital uno o dos días después, no había manera de salvarme", comenta. Su hermano mayor, también infectado, no tuvo tanta suerte.
Si la supervivencia de Hung es asombrosa, también la forma en la que contraería la enfermedad. En una visita a su pueblo natal en las afueras de Hanoi, él y su hermano compartieron un pudín hecho con sangre de pato cruda, un manjar vietnamita. El pato debe haber estado infectado con el H5N1, porque el hermano de Hung caería abatido por la gripe unos días después. Hung no se enfermó durante más de dos semanas, no hasta el día después de la muerte de su hermano.
Es demasiado tarde para que el pudín sea la causa, cree Tim Uyeki. Hung probablemente se infectó mientras cuidaba a su hermano en sus últimos días.
También se cree que el H5N1 pasó de una niña que murió de gripe aviar en Tailandia a su madre y a su tía, quienes la cuidaban. Hay sospechas de otros casos de transmisión entre humanos. Un paso más allá de la primera víctima es todo lo que el virus parece haber conseguido. Hasta ahora, la transmisión continua (propagación en cadena de la gripe común) no está en su repertorio.
Podría adquirir esa capacidad por sí sola, mutando. O intercambiando genes con un virus de la gripe humana. Eso podría producirse en una persona infectada que también contrajera la gripe común, o en el clásico recipiente de mezcla de la gripe, el cerdo. Aun así, nadie sabe si el intercambio diera lugar a una pesadilla o a un fracaso del virus.
Epidemiólogos en el CCPEEU y en los Países Bajos esperan descubrirlo con antelación, mediante la reproducción artificial de nuevos virus. En laboratorios de alta contención están mezclando y combinando deliberadamente genes del H5N1 y de los virus de la gripe humana. Luego evaluarán los híbridos para ver si alguno de ellos ha heredado tanto la virulencia del virus aviar, como la capacidad de propagación del virus humano. En efecto, están tratando de crear una cepa de gripe pandémica en el laboratorio.
Algunos críticos piensan que es una imprudencia, pero Erich Hoffmann, un investigador de San Judas que ayudó a desarrollar una técnica de ingeniería genética para la gripe, dice que los experimentos son clave para saber lo que eventualmente podríamos enfrentar. "Básicamente es el mejor simulacro que se puede tener en el laboratorio de lo que posiblemente podría suceder en la naturaleza".
"Si tenemos suerte", dice, "todos los virus híbridos serán inofensivos o no prosperarán", sugiriendo que el H5N1 puede que nunca genere una pandemia. "Por supuesto que eso sería una buena noticia", dice Hoffmann. ¿Y si las noticias no son alentadoras? Entonces los científicos que monitorean el H5N1 en Asia tendrían una pista sobre lo que hay que vigilar, ya que los cambios genéticos del virus podrían indicar que se avecinan grandes problemas.
Ese conocimiento podría ganar algo de tiempo de advertencia, pero nadie quiere sentarse a esperar que el H5N1 dé el primer paso. Y todo el mundo está de acuerdo en la mejor manera de prevenirlo: Erradicando el H5N1 en las aves de corral, para que la gente no pueda contagiarse. Es simple, pero no es fácil.
Howard Wong, un veterinario de alto rango en Hong Kong, sabe lo que esto implica. Desde 2003, tras múltiples brotes y dos sacrificios masivos de aves de corral, el territorio ha mantenido sus granjas y mercados libres de H5N1. Wong está orgulloso de su logro. "Fuimos muy felices en 2004, cuando estaba en todas partes, y logramos contenerlo".
Algunas de las medidas que Wong y otros funcionarios tomaron son: vacunar a todos los pollos de Hong Kong contra el H5N1; examinar regularmente a los pollos, las aves domésticas, e incluso a las aves silvestres; cerrar los cientos de puestos de aves de corral dos veces al mes para desinfectarlas; e inspeccionar obsesivamente las granjas y los mercados. "Estamos casi al límite de lo que podemos hacer", dice Wong. "Es como sostener este muro, y este muro se hace cada vez más pesado".
Como dice un funcionario de asistencia en Vietnam, Hong Kong es rico, y una isla. Vietnam no es ninguna de las dos cosas. En el invierno de 2003-2004, el H5N1 estalló en la mayoría de las 64 provincias del país. Para combatirlo, el país sacrificó decenas de millones de pollos. La epidemia pareció disminuir, y en marzo el gobierno declaró la victoria.
Pero a finales del 2004 la gripe aviar volvió a retumbar, infectando aves y personas a lo largo del país. A simple and direct question for you today — Pastor John, over the years, what things have been most apt to steal your joy?
My kids, my marriage, and my soul in response to my kids and my marriage. If things are sweet at home, you can stand almost anything in the ministry. At least, that is my experience. The hardest battles for me have been emotional battles within my family. Living by the Spirit or trying to minister by the Spirit doesn’t prevent hard relational things from happening in the ministry. It just gets you through them and gets you through them with hope, and, in the end, with joy in the sustaining grace of God.
Six Counsels for Joyful Family and Joyful Ministry
Now here are some implications of that answer. I mean, that is pretty weighty to say the biggest joy-stealers in the ministry are not deacons. They are not elders. They are not counseling issues. They are not people who leave the church. They are not notes you get in the mail from a disgruntled parishioner. Those problems are just nowhere near as emotionally depleting as conflict in a marriage, or your kids that are disappointing you, or you are disappointing them, or there is conflict. So, here are some implications.
1. Authentic Christian marriages empower ministry.
Investment in a happy, Christ-honoring marriage is not separate from ministry. For me, it was part of the power of ministry. And I don’t just mean that my ministry is legitimated because elders are supposed to have well-behaved homes. That is not what I mean. I mean the actual motivation for doing ministry rose and fell with authenticity of Christian living at home.
And so it wasn’t merely, Oh, do I qualify? like some external rule that I had to meet. It was rather, Can I survive? You know? Is the burden that I feel at home depleting so much energy I won’t have anything for Sunday morning? So, to invest in the family was to invest in my survival, not just my qualifications.
2. Hypocrisy at home undermines spiritual authority in the church.
Investing in the joy and welfare of the children — not just the marriage, but in the children — was not separate from ministry, because it was part of the power; a sense of authenticity rose and fell with them as well. Hypocrisy is not only a great sin, it is a great weakener. If you feel like you are inauthentic with your kids — like you are one thing in the pulpit, and your kids see you as another thing — it is going to be very hard to keep going. It is a power-depleter. It is an underminer of real spiritual authority in the pulpit and power in people’s lives.
3. Commit your family’s struggles to elders and believing friends.
When struggles inevitably come at home — and they will come with a wife and children — don’t go underground with them. Entrust yourself to the elders and to close friends. Enlist prayer. Get the counsel you need. This is for the sake of ministry, not just for the family. It is all woven together.
4. Weather the storm.
When the storms come, don’t quit. Satan wants you to quit. He is as active as anybody in this thing, and he wants you out of the ministry — or at least out of fruitfulness. He wants you paralyzed with discouragement. Tell him to go to hell, which is where he belongs, and then bank on the promises of God that “he who is in you is greater than he who is in the world” (1 John 4:4). That promise has been very precious over the years as I have gotten in Satan’s face about his temptations.
5. Do not encourage hypocrisy in your family.
When storms come — and they will — don’t manipulate your kids or your wife by telling them, “Now, if you don’t behave, you are going to disqualify me from ministry.” That is just an absolutely wrong thing to say because that kind of obedience isn’t obedience, right? You don’t want your kids or your wife thinking about your ministry that way. You shouldn’t ever jerk them around like that or use your role as a kind of lever to try to get their hearts to be right with God.
That is just counterproductive. It is not going to work. There are reasons to obey as children and reasons to walk in joyful holiness, and that is not one of them. There ought to be other reasons. If a kid is just containing his worldliness to keep daddy’s job, that kid is going to explode someday and do more damage than if he had been honest. So dad shouldn’t encourage that kind of hypocrisy in his kids.
6. Pray earnestly for God’s help.
And the last thing I would say is, Cry out to the Lord for help. That is what I did over and over again. He won’t let you be tested beyond what you are able, but with the testing — you know, we usually translate that temptation, and we almost always think of sex. The word pierosmos means testing as well as temptation. “He will not let you be [tested] beyond your ability, but with the [testing in the family] he will also provide the way of escape, that you may be able to endure it” (1 Corinthians 10:13).
I have seen light at the end of tunnels that seemed impossibly dark. So that is God’s answer to that promise in 1 Corinthians 10:13. He will make an opening at the end of this tunnel. Trust me. Hold on. Don’t doubt me in the dark. I am a God of light, and I will be with you.

Solamente te lleva a través de ellos con esperanza, y al final con alegría en la gracia sustentadora de Dios.
Seis Consejos para un Ministerio y una Familia Feliz
Aquí hay algunas implicaciones de esa respuesta. Es decir, es bastante serio decir que los mayores ladrones de alegría del ministerio no son diáconos. No son mayores. No son consejeros. No son personas que abandonan la iglesia. No son notas que recibes por correo de feligreses descontentos. Estos problemas no son emocionalmente desgastantes como lo es el conflicto en un matrimonio, o la decepción de un hijo, o tú decepcionarlos a ellos, hay conflicto, así que aquí hay algunas implicaciones.

La hipocresía no solo es un gran pecado, es una gran debilidad. Si sientes que no eres auténtico con tus hijos (como si fueras una cosa en el púlpito, y tus hijos te ven como otra cosa) va a ser muy difícil seguir adelante. Es un destructor de poder. Es un reductor de la verdadera autoridad espiritual en el púlpito y del poder en la vida de las personas.
3. Encomienda las luchas de tu familia a los ancianos y a los amigos creyentes.
Cuando las luchas inevitablemente llegan a casa (vendrán con una esposa e hijos) no vayas a la clandestinidad con ellos. Confía en los ancianos y en los amigos cercanos. Pedir en oración. Busca el consejo que necesitas. Esto es por el bien del ministerio, no solamente por la familia. Está todo entrelazado.
4. Afrontar la tormenta.
Cuando llegue la tormenta, no te rindas. Satanás quiere que renuncies. Es efectivo como nadie en esto, y te quiere fuera del ministerio, o al menos de fructificación. Quiere que te paralices con el desánimo. Dígale que se vaya al infierno, que es donde pertenece, y luego confíe en las promesas de Dios de que "él que está en ti es más grande que el que está en el mundo" (1 Juan 4:4). Esa promesa ha sido muy valiosa a lo largo de los años, ya que me he enfrentado a Satanás por sus tentaciones.
5. No fomente la hipocresía en su familia.
Cuando llegue la tormenta (y llegará), ni manipules a tus hijos ni a tu mujer diciéndoles, "Si no te comportas, me vas a desacreditar en el ministerio". Eso es algo absolutamente erróneo, porque ese tipo de obediencia no es obediencia, ¿verdad? No quieres que tus hijos o tu esposa consideren tu ministerio de esa manera. No deberías nunca engañarlos así o usar tu posición como una especie de ventaja para tratar de que sus corazones estén bien con Dios.
Eso es simplemente contraproducente. No va a funcionar. Hay razones para obedecer como niños y razones para caminar en alegre santidad, y esa no es una de ellas. Debería haber otros motivos. Si un niño controla su mundanalidad para mantener el trabajo de papá, ese niño va a explotar algún día y hacer más daño que si hubiera sido honesto. Así que el padre no debería alentar ese tipo de hipocresía en sus hijos.
6. Reza fervientemente por la ayuda de Dios.
Y la última cosa que diré es, clama al Señor por ayuda. Eso es lo que yo hacía una y otra vez. No te pondrá a prueba más allá de lo que puedas, pero con las mismas pruebas, ya sabes, normalmente interpretamos esa tentación, y casi siempre pensamos en el sexo. La palabra pierosmos significa prueba así como tentación. "No os dejará ser [puestos a prueba] más allá de vuestras posibilidades, sino que con la [prueba en la familia] os dará también la vía de escape para que podáis soportarlo" (1 Corintios 10:13).
He visto luz al final de túneles que parecían completamente oscuros. Así que esa es la respuesta de Dios a esa promesa en 1 Corintios 10:13. Él hará una salida al final de este túnel. Confía en mí. Aguarda. No dudes de mí en la oscuridad. Soy un Dios de la luz y estaré contigo.
