Marqueurs du risque cardiovasculaire :
Cholestérol total, cholestérol HDL, rapport cholestérol total/HDL, cholestérol LDL, triglycéride, CRP, acide urique sont dans les limites de la normalité.
Dosages vitaminiques :
Acide folique, vitamine B12 sont à des niveaux physiologiques. Le niveau de la 25-hydroxyvitamine D est modérément déficitaire à 40 nmol/l.
Dosages hormonaux :
La TSH à 1.30 mUI/l indique un fonctionnement adéquat de la glande thyroïde, confirmé par la normalité de la valeur de la T4 libre. La production de testostérone totale ainsi que celle de la gonadotrophine LH sont dans les limites de la normalité.
Marqueurs des métabolismes osseux :
La concentration du télopeptide C-terminal (marqueur de la dégradation osseuse) est normale. Ainsi qu’il est dit plus haut, la concentration de 25-hydroxyvitamine D est déficitaire à 40 nmol/l.
Marqueurs tumoraux :
La concentration des marqueurs de cancers digestif/glandulaire (CEA), hépatobiliaire (AFP), pancréatique (CA 19-9) et prostatique (PSA) est en deçà du seuil pathologique.
Marqueurs sérologique :
La recherche d’antigène et d’anticorps spécifique du virus de l’hépatite B, du virus de l’hépatite C et du virus de l’immunodéficience humaine (HIV) est négative.
Examen des urines :
L’examen du sédiment urinaire est sans particularité et l’excrétion urinaire de protéine et microalbumine est physiologique.
Radiologie :
CT-scan thoraco-abdomino-pelvien :
A l’étage thoracique, on n’observe ni nodule ni masse d’allure suspecte ni infiltrat pathologique. Il existe un discret épaississement diffus des parois bronchiques. Les artères pulmonaires sont bien perméables. Il n’existe ni épanchement pleural ni péricardique ni d’adénopathie hilaire ou médiastinale.
A l’étage abdomino-pelvien, le foie est de taille et de morphologie normales sans lésion focale. Les veines hépatiques et le tronc porte sont bien perméables. Les voies biliaires intra et extra-hépatiques, la vésicule biliaire, la rate, le pancréas, les glandes surrénales, les reins, les chaînes lymphatiques rétro-péritonéales, mésentériques et pelviennes, la vessie et la prostate ne présentent aucune modification de nature pathologique. On visualise quelques diverticules sigmoïdiens mais sans signe de complication.
Score calcique coronarien :
Le score d’Agatston est calculé à 3, correspondant à la totale absence de dépôt de cholestérol calcifié significatif sur l’ensemble du réseau coronarien.
Épreuves fonctionnels cardiaques
Echocardiographie :
Les cavités cardiaques et les parois myocardiques sont de taille normale. Les valves mitrales aortiques, tricuspides et pulmonaires sont d’aspect et de fonction normaux. Le septum interauriculaire est d’aspect normal sans shunt visible au Doppler couleur. La contractilité segmentaire et globale du ventricule gauche est conservée avec une fraction d’éjection systolique normale calculée à 64%.
Test d’effort :
L’examen, complet en termes d’effort et de fréquence cardiaque, est cliniquement et électriquement négatif malgré un segment ST légèrement oblique ascendant. Il existe une hypertension artérielle modérée à l’effort avec un léger retard d’adaptation hémodynamique en phase de récupération. On note une extrasystole ventriculaire sans autre arythmie significative. La capacité fonctionnelle est bonne à 11.7 METs pour une CMT entre 7.7 et 10.0.
Commentaires et recommandations :
Votre état de santé actuelle est excellent.
Votre examen clinique est dépourvu d’anomalie. Aucun des naevi présents sur votre revêtement cutané ne présente pour le moment de caractère suspect. Vos analyses sanguines et urinaires ne détectent aucune altération biologique significative en dehors d’un léger déficit en vitamine D3 que vous pourriez corriger pendant les mois d’hiver par une supplémentation alimentaire en cette vitamine.
L’examen par CT-scan des organes contenus dans votre cavité thoracique est entièrement normal.
L’examen scanographique des organes contenus dans votre cavité abdomino-pelvienne ne révèle aucune modification de nature pathologique. On note simplement la présence de quelques diverticules (petites hernies de la muqueuse colique) au niveau du côlon sigmoïde, sans signe d’inflammation (pas d’évidence de diverticulite).
Votre score calcique coronarien est pratiquement nul, indiquant une totale absence de dépôt de cholestérol calcifié sur l’ensemble de votre réseau coronarien.
Cet excellent état coronarien est encore confirmé par la normalité de votre échocardiographie et de votre test d’effort qui montre tout au plus une légère augmentation de votre pression artérielle lors de l’effort physique avec une récupération un peu lente. Radiololgy:
Thoraco-abdomino-pelvic CT-scan:
Neither a nodule nor a suspicious looking mass or pathological infiltration observed on the thoracic level. There is a slight diffuse bronchial wall thickening. The pulmonary arteries are permeable. There is neither pleural or pericardial effusion nor hilar or mediastinal lymphadenopathy.
On the abdomino-pelvic level, the size and morphology of the liver is normal without focal lesions. The hepatic veins and the portal trunk are permeable. The intra and extrahepatic bile ducts, the gall bladder, the spleen, the pancreas, the adrenal glands, the kidneys, the retroperitoneal, mesenteric and pelvic lymphatic chains, the bladder and the prostate do not indicate any pathological changes. We observed a few sigmoid diverticula but without any signs of complications.
Coronary Calcium Score:
The Agatston score result is 3, related to the total absence of significant calcified cholesterol deposition within the entire coronary network.
Cardiac function tests
Echocardiography:
The heart chambers and myocardial walls are a normal size. The aortic, tricuspid and pulmonary mitral valves are normal in appearance and function. The interatrial septum is normal in appearance with no shunt visible on the colour Doppler. The segmental and global contractility of the left ventricle is maintained with a normal systolic ejection fraction at 64%.
Stress test:
The examination, complete in terms of exertion and heart rate, is clinically and electrically negative despite a slight, oblique ST segment elevation. There is moderate arterial hypertension on exertion with a slight delay in hemodynamic adaptation in the recovery phase. Ventricular extrasystole observed without any other significant arrhythmia. Functional capacity is good at 11.7 METs for a TAC of between 7.7 and 10.0.
Comments and recommendations:
You are currently in excellent health.
Your physical examination was devoid of any abnormalities. None of the nevi on your skin are currently suspicious looking. Your blood and urine tests did not detect any significant biological alteration apart from a slight deficiency in vitamin D3 that you could correct during the winter months with D3 dietary supplements.
The CT scan of the organs in your chest cavity is completely normal.
The CT scan of the organs in your abdominopelvic cavity does not show any pathological changes. We only observed a few diverticula (small hernias of the colonic mucosa) in the sigmoid colon, without any sign of inflammation (no evidence of diverticulitis).
Your coronary calcium score is practically zero, indicating a complete absence of calcified cholesterol deposition in your entire coronary network.
This excellent coronary state is again confirmed by your normal echocardiography and your stress test which shows at most a slight increase in your blood pressure during physical exertion with a somewhat slow recovery. Due to this abnormality, you should check your blood pressure from time to time and make sure that it is below the limit of normal blood pressure (145/85 mmHg).
Proposed Treatment:

Votre fonction rénale était normale avec une créatinémie à 87 µmol/l et une clairance de la créatinine évaluée à 84 ml/min/1.73 m2. Votre niveau d’urée sanguine était discrètement augmenté à 10.7 mmol/l, reflétant votre niveau d’hydratation. L’examen de votre sédiment urinaire était sans particularité et votre excrétion urinaire de protéines et de microalbumine était physiologique.
Votre niveau d’acide urique était à la limite supérieure de la norme (412 µmol/l), reflétant une alimentation trop riche et trop carnée.
Sur le plan hormonal, votre fonction thyroïdienne était normale avec une TSH à 0.70 mUI/l et une T4 libre à 12.2 pmol/l. La recherche d’autoanticorps anti-thyroglobuline et anti-thyroperoxydase était négative. Le niveau de thyroglobuline était normal.

On constatait simplement une altération discale dégénérative en L3-L4 et L4-L5, sans protrusion discale significative et sans compression d’une racine émergente. Une évaluation diagnostique de vos douleurs nocturnes lombaires nécessite un examen de l’ensemble des coupes IRM effectuées de même qu’une IRM du rachis dorsal, une éventuelle lésion pouvant être plus haut située.

Please find attached, the results of the blood and urine tests performed during your visit to...... (Switzerland) on February 25, 2021.
Your renal function was normal with a creatinine level of 87 µmol / l and an estimated creatinine clearance of 84 ml / min / 1.73 m2. Your blood urea levels had increased slightly to 10.7 mmol / L, reflecting your hydration level. Your urine sample test was unremarkable and your urinary protein and microalbumin excretion was physiological.
Your uric acid levels were at the upper limit of normal (412 µmol / L), reflecting a diet that was too rich and too high in meat.
On the hormone front, your thyroid function was normal with a TSH value of 0.70 mIU / l and a free T4 value of 12.2 pmol / l. The test for anti-thyroglobulin and anti-thyroperoxidase autoantibodies was negative. Your thyroglobulin levels were normal.

Veuillez trouver ci-joint le résultat des examens pratiqués lors de votre dernière visite le 17 mars 2021.
Votre examen clinique était dépourvu d’anomalie si l’on excepte la présence de kératoses actiniques et même de petits épithéliomas basocellulaires au niveau de la face dorsale des deux mains. Votre poids était de 85 kg (versus 92 kg en 2018). Votre pression artérielle était normale (105/75 mmHg) avec une fréquence cardiaque régulière de 65 bpm.
Vos analyses sanguines incluant paramètres biologiques classiques, marqueurs métaboliques, marqueurs du risque cardiovasculaire, dosages vitaminiques, dosages hormonaux, marqueurs du métabolisme osseux, marqueurs tumoraux et marqueurs sérologiques retrouvaient quelques discrètes altérations d’ordre métabolique à savoir :
une légère augmentation de votre glycémie à jeun (6.30 mmol/l).
une discrète augmentation de votre niveau de ferritine (339 µg/l).
une légère élévation de votre niveau de « mauvais cholestérol » LDL (3.58 mmol/l).
Ces minimes anomalies devraient se corriger spontanément en maintenant votre poids actuel et en poursuivant votre entraînement physique régulier.
L’examen par ultrason des organes contenus dans votre cavité abdomino-pelvienne montrait un foie de taille normale, de contours réguliers comportant un kyste biliaire banal, infracentimétrique, situé à la pointe du lobe droit. Please find attached, the results of the tests carried out during your last visit on March 17, 2021.
Your clinical examination was devoid of any abnormalities except for the presence of actinic keratoses and some small basal cell epitheliomas on the dorsal side of both hands. Your weight was 85 kg (versus 92 kg in 2018). Your blood pressure was normal (105/75 mmHg) with a regular heart rate of 65 BPM.
Your blood tests including classical biological parameters, metabolic markers, cardiovascular risk markers, vitamin levels, hormone levels, bone metabolism markers, tumour markers and serological markers showed some moderate metabolic changes, namely:
A slight increase in your fasting blood sugar (6.30 mmol / l).
A slight increase in your ferritin levels (339 µg / l).
A slight increase in your LDL “bad cholesterol” levels (3.58 mmol / l).
These small irregularities should correct themselves if you maintain your current weight and continue your regular physical training.
The ultrasound examination of the organs in your abdominopelvic cavity showed a normal sized liver, regular contours with a banal, infracentimetric biliary cyst located at the tip of the right lobe. The intra and extrahepatic bile ducts, intrahepatic vessels, gallbladder, spleen, pancreas, kidneys, abdominal aorta, iliac arteries, bladder and prostate gland showed no pathological changes.
Your stress test, complete in terms of exertion and heart rate, produced neither clinical manifestations nor electrical changes, suggesting insufficient coronary perfusion of the heart muscle. Your blood pressure and heart rate adapted perfectly on exertion with a good recovery time of 3 minutes. Your functional capacity was good at 10.8 METs for a theoretical average capacity of between 6.6 and 8.6. No significant arrhythmias were observed on exertion, except for a few rare atrial extrasystoles.
