Det er en pligt for regionen til at overtage og indlægge de patienter, som ikke kan sendes hjem til deres eget hjem, fordi de ikke kan klare sig selv.

These are in fact psychiatric patients, whose treatment has been completed at a large specialist hospital, and the patients must be moved back to their normal lives if possible, while the beds and hospital places are needed for others in active treatment.

post-discharge care?

The text is actually a PP presentation, and very fragmented, but for once there is space on the slides for a meaningful translation of reasonable length.
It does need to be concise and as far as possible self-explanatory, however.

discharge planning Discharge planning is another expression I have found.
Something like:
"The regional authority has an obligation to take over patients for discharge planning, if they are unable to manage in their own homes."

I would rather go for pre-discharge - as I have a feeling that final discharge is not a fact until the patient returns to his home.

"Victoria is a pre discharge ward. Patients need to be medically stable prior to arrival on Victoria Ward. Discharge planning for patients with complex social and medical problems. "
