I tried to rework this in several ways but the source is quite confusing and this is is the only way it would make sense to me.
I also think the punctuation in the Italian doesn't help.

: Maybe "As in the ancient definition of Middle East" could also be correct? Meaning the author uses "panorientale" (referring to the whole East) as the macro region the term Middle East referred to originally
