The title of a personal document - it looks very muck like a birth certificate. Norstedts Engelska Ordbok has it as 'hist., se personbevis'. Is it an obsolete form used for 'personbevis'?

Åldersbetyg is not exactly obsolete (my dictionary from -73 lists åldersbetyg but not personbevis).
Today, I think personbevis is generally used instead of åldersbetyg.

Perhaps it is so in Norway. In Sweden, it is "a birth certificate, which is a document issued by the local registrar". There it is issued by the tax authorities, and you need an åldersbetyg/personbevis to prove your identity in order to get a card (ID-kort).
Stora svenska ordboken (Norsteds 1986): "intyg med uppgifter om viss persons namn, födelsetid och civilstånd m. m. ~ krävs bl. a. när man söker pass"

proof of age card
There are lots of references on the Internet for this. It is not the same as a birth certificate, which is a document issued by the local registrar (usually A4 or A5 size paper). It is, in fact, a card.

Proof of identity
I live in the States and I've never even heard the term "Proof of Age Card" and "Birth Certificate" isn't the correct translation.
Example sentence:
Åldersbetyg krävs för att köpa snus.
Proof of identity is required to buy snuff.
