In the 1980s, multicentre, controlled and randomised studies (ECST, NASCET) started to delimit the type of patient (symptomatic or assymptomatic) and the degree of stenosis at which surgery could offer a more favourable quotient.
In 1980 the first angioplasties in patients with stenosis were performed.The challenge of the new techniques (angioplasty at onset and angioplasty + carotid stent subsequently) is to overcome the rates of morbidity and mortality in surgery.
A randomised study, the CAVATAS,(1) with 504 patients treated between 1992 and 1997, comparing endovascular treatment to carotid endarterectomy, concluded that the greater risks and the effectiveness in preventing stroke over 3 years are similar in both groups.
