As mentioned above, Smt is the equivalent of Mrs.

As for the other abbreviations, I did a little search on the internet and found a (probably) similar document to the one you're translating (it would have helped of course if you had given us more context) - http://cbi.nic.in/al1.htm

From that document, I can guess that w/o stands for "wife of", s/o for "son of" and r/o for "resident of" (since that last one is followed by an address, not a name). Can you confirm if that makes sense?
