It's a PPT presentation, so there's not very much in the way of sentences...
Situation: Incidents of non-compliance or fraud arise at a company. Complaints are made to the compliance office, which creates a "Fallakte". The case manager coordinates the investigation and documents the "Fallbegleitbogen".
Can anything think of anything for this? I think the "Begleit" bit is kind of superfluous in English but I'd like to hear other people's opinions. Thanks in advance!

Although "case report form" is an established term in the med/pharma field (with a different German translation - "Prüfbogen"), it might fit here, too.

The "-begleit-" part tells me that it is a sheet that goes with (=accompanies) the case record ("Fallakte")

Personally, I would prefer "complaint documentation"!
