You need to consider both DD and DT together, and the fact that they are followed by a date. The only answer that makes sense is Michal's. Date of dictation and date of transcription.

agree liz askew : DD = date of discharge, I believe.//Oh yes, have just seen date of dictation,,,yes this would make sense. There was me thinking they were medical abbreviations!

Many thanks Liz. Report was written in the US. I am afraid DD means not "died", as I am translating for the patient :-).
