Colloquially, we would use this expression.
We omit the words 'I am' and 'and'.

Mabuti ako, ikaw?

Mabuti naman ako, at ikaw?

It is an expression that cannot be translated literally, but the pronoun "I" can be translated as "ako," "fine" can be translated as "mabuti" or "maayos," referring to one's condition, and throwing back the question to the other by using the pronoun, "ikaw", meaning "you." "Ikaw" is used when the person you are talking to is familiar to you. We use "kayo" for formal conversation (Mabuti po naman ako, at kayo?).
