Rita raises a point I'm guessing that your client asked you to translate the article titles. Otherwise they are not translated. WHO style (Vancouver style) puts title translations in square brackets at the end of the reference if they are required.

Liz, "major" is used as the collocation for "factor" (google "major vascular risk" and you'll find it actually refers to "major vascular risk factors"). I'm quite positive the correct term is primary (actually, "primary coronary risk" is highly frequent as a collocation), but then again, I don't specialize in medicine and maybe someone can give further details about this.

I do not think "major" is appropriate, because the meaning here is rather "essential". I agree with primary as the medical word.

Due to the insistence of the other answerers in the meaning of primary, I've continued searching, but really, without further context it's difficult to reach a conclusion (you'll have found that there are no hits for the article in Spanish).

However, it does seem that another possibility might be:
Primary risk factors for vascular disease (but you have to add disease/disorder/event, or something, in English).

The trouble is I searched for "primary vascular risk" and found no references on UK sites. It could be "major"??
