Da gibt es doch sicher so etwas wie eine Standardformulierung, weiß jemand eine solche?

rewrite

I'd split it into parts:

"The patient was discharged[...] The GP will assume further care of the patient.

Just because the usual statement is 'discharged home'. Never read 'discharged into GP care'.
