First of all, I don't think this concerns good Chinese. I doubt then the author means "所提供资质(上注明)的户名" OR “经/有资质证明的户名”。 There come therefore two expressions as titled in English for your options. You can choose according to your actual detailed context.

Provide Accepted Username (or Account name)
This 提供资质户名should not only be considered, but the phrase 资质户名全称 should be considered as a whole. Directly translating 资质 as "qualified" in English doesn't completely make sense in this context. When you are register for an account, that account name is generally called "accepted" or is just a "username" or "account name" with no adjective describing that your chosen name was accepted or is "qualified". This sounds smoother. But I also agree with Adsion Liu above, it doesn't sound like good Chinese.
