: "Certificate of Facts" a szabványos neve az USA-ban. A Notarial nem kell, mert nem része a megnevezésnek (nincs benne, hogy "közjegyzői"), és nemcsak közjegyző készítheti. Többet referenciaként felteszek.

: Witness Statement (UK) / Witness Deposition (US) / Tanúvallomás (HU) is a statement summarising the oral evidence that a witness will give at trial. That is not the term in question, "Ténytanúsító jegyzőkönyv" (which I believe is "Certificate of Facts").

Az USA-ban ez a szabványos neve. Megadok két példát, az egyik egy földmérő által készítendő jegyzőkönyv sablon.ja, a másik jogosítvány bevonásával kapcsolatos, a harmadik (amit ide másolok) pedig választásokkal kapcsolatos, nem sablon, hanem egy tényleges dokumentum.
