If you want something brief, this might work. The elliptical expression "self-employed contributions" is perfectly standard in English-speaking countries (see http://www.google.es/search?hl=en&biw=991&bih=541&q="self-em... ), and I think we could borrow it, just adding "receipt for". This does involve repeating "contributions", and I suppose that could be avoided either by using "self-employed payments" (there are different kinds of self-employed payments, but in context this will obviously refer to Social Security contributions), or even cut it right down to "receipt for the self-employed", which in context is sufficient, I think, and is perhaps the neatest solution.

As these payments are usually done by bank transfer, it would be the proof of bank transfer. In Spain individual receipts are mailed when someone makes or receives a transfer. The motive for the transfer "concept" indicates that it was a payment to the SS system.

This is what is often referred to as the cupón, which so many of us have to fork out for each month! If you have a direct debit set up, you won't see it physically, and the bank operation is used as proof of payment. If you do it manually each month, you get a two-part slip, and on payment, retain a stamped half: http://www.seg-social.es/prdi00/groups/public/documents/bina...
