Italian term

medico informatore

Italian to English Medical Medical (general)
On a consent form for an operation - where the doctor signs: "Firma del medico informatore". From the rest of the consent form I understand this term to mean the doctor who has explained the procedures and risks of the operation to the patient, but any advice on the correct term to use in English would be much appreciated.
