I'm with Thomas to a certain extent but I think it's preferable to use words with a positive connotation - so "remembered" is better than "forgotten", making "Be seen, be remembered" a better option.

Get noticed, be remembered

A suggestion, i.e. if you don't get noticed (in the first place), you won't or can't be remembered.

Other options:
Get noticed and be remembered
Be noticed (and) be remembered
Get noticed (and) stay noticed

Your own suggestion "be seen or be forgotten" also seems quite common, but perhaps "be seen and not forgotten" might be another alternative?
