Provision of Services Certificate

I don't think there's much wrong with Ruth's suggestion but I think options based on "provided/provision" are more common and perhaps sound a bit more modern.

I think it depends on whether "Certificado de prestación de servicios" is treated as a single, proper entity. If so, with my suggestion, you'd probably have to say something like "Provision of Services Certifcate of work in/for [company]"
Example sentence:

If you are registered as a UK-practising member with the RCVS and you are a community-rights-entitled person who wants to provide services in other EU member states, you will need to ask the RCVS to provide you with a Provision of Services Certificate...
