About health insurance coverage

Good Cause and Hardship Request
If you are on a VPharm program and are having problems enrolling in or getting coverage from your new Part D plan, you should contact the Part D plan (or ask someone you trust to contact them on your behalf).
If that does not work and not having your prescription is likely to cause you serious harm, you can submit a ***Good Cause and Hardship Request*** to Vermont for help obtaining your medications until the problem with your Part D plan is resolved.

difficulté(s) financière(s)

This is the meaning, I am absolutely certain. However, you would have to decide how to integrate it into the title of the aid program you mention (causes méritantes et difficultés finanancères for example)

Hardship payments and Jobseeker's Allowance

A hardship payment is a reduced amount of Jobseeker's Allowance which you may be able to get if you have been refused Jobseeker's Allowance, or your benefit has been stopped, because of doubts about whether you are available for and actively seeking work. You may also be able to get them if:

you are waiting for the Jobcentre Plus office to decide whether you meet the jobseeking conditions
you have been sanctioned (see under Jobseeker's Allowance and sanctions)
your JSA has been stopped for a fixed period following a benefit fraud offence.
If you want more information about hardship payments, and how to apply for them, you should ask in the Jobcentre Plus office for form JSA 10, 'Jobseeker's allowance hardship application'.

Here's another example : http://dpac.uk.net/2013/05/hardship-paymentsbudgeting-loans-...
I am insisting a bit, as hardship can (as you will see on Google) also mean difficulty in general (there are hardship payments for expats working in dangerous countries for example, and this is like a bonus on their salary. The context you give indicates that it referes to people who are in temporary difficulty paying for their medication
