Учитывая контекст, возможно, речь о том, что персонал должен работать с клиентами-больными ВИЧ осторожно, чтобы их помощь и поддержка не навредила таким клиентам или не оттолкнула ( не отпугнула) их.

Думаю, что disenfranchise здесь употребляется в переносном смысле.
