Gentlemen, I left the hotel on time but had a flat tire on the way. While I was changing the tire, a lug nut fell out of my hand and rolled into the middle of the busy highway. Not thinking, I stepped out and picked it up. When I stood up, I realized that there was a thirty-ton logging truck bearing down on me at approximately seventy-five miles an hour. It hit me head on and ran over me.

Explanation:
Понятно, что ударил передком. Но здесь head on явно используется для усиления. Думаю, переводить "в лоб" не стоит. Тут скорее перед глазами появляется баран, который бьет лбом с разбега.
