A verbal warning will be given for the first and second broken appointments and documented in the family’s record. Should a third appointment be broken, the patient and family members will be dismissed from the practice. A dismissal letter will be mailed home for the patients of record
I am thinking of these three translations:
سوف يتم رفض تقديم الخدمات الطبية للمريض وأفراد عائلته في عيادتنا

سوف يُطرد المريض وأفراد عائلته من عيادتنا

سوف نتوقف عن تقديم الخدمات الطبية للمريض وأفراد عائلته في عيادتنا
