Sadly, not handwritten @neilmac if it were handwritten it may be excusable, though sadly not.

Is it handwritten? Doctors are traditionally notorious for slipshod script...

From the context, I'd expect it to be the patient's name, or something like "the patient".
Helena Chavarria Oct 6, 2016:
I definitely wouldn't guess. I'd mention it to the client.

Ambito Thanks David, "ambito" could work.
I'm not sure I would risk a guess though, would it not be safer to refer back to the client?
I appreciate your help 😊

AMARILICO In my case, I would leave it blank with a suggestion that it may be environment (AMBITO)

Mistake? Thanks Helena, I had also suspected it was a mistake, though it's always good to get a second opinion.
Helena Chavarria Oct 6, 2016:
I think it has to be a mistake.

@David 'la funcionalidad de AMARILICO tanto a nivel escolar como familiar'

a literal translation would be

'the functionality of yellow fever at both an educational and family level.' It sounds it bit strange to me.
