Motivo del Ingreso

Non-PRO Spanish to English Medical Medical (general) Patient Report from Spain
This is a very common term but I'm having a bit of trouble with the wording, this is part of a medical report that's included in a contract.
It's the reason a patient visits the hospital, reason of entry, etc.
I'm not specialized in medical translations, so I'd appreciate the help of any of you who are familiar with this.

Reason for hospitalization

Not a specialist either but I would hazard that this is standard technical as well as layman's parlance in this context.

Ah, and perhaps I should stress that the meaning of the Spanish is NOT just a hospital visit, it is talking about the reason the patient has been admitted to hospital.

And following that thought, two alternatives: "reason for admission [to hospital]", "reason for hospital admission".
