what this form says is:
no more paid days off (/vacation) are pending / due - and it's an agreement between the employer and the employee, NOT a one way "avis/notice/...".
The fact that these days of vacation have been paid in-lieu is not part of the form, but the reason why this agreement was reached.

: as far as I can understand what "attribuer" means what you would make them sign would make them confirm that they have been allocated the right number of days of vacation. This form is about something else.
