: It seems unlikely that in the middle of a section of patient identifying information there would be a mention of (or need to mention) something about a non-profit association. Why would they mention this in this context?

: For this context, this makes no sense. This is a patient's personal information document, so it is relating to the patient, not an organization.

Otra posibilidad es que el informe forme parte de un expediente, donde la fórmula número de expediente AC-xx/xxx... resulta común -aunque su expresión como No AC me temo que no.
