PTU is a rare neuro‐ophthalmological disorder with onset in infancy. It is characterized by repeated episodes of sustained or intermittent upward gaze deviation, conserved horizontal eye movements, and down beating nystagmus in attempted downgaze.

In one case the nystagmus was a right beat horizontal jerk nystagmus, accompanied by upward gaze deviation but retained consciousness (1455), in the other there was right gaze deviation, loss of consciousness and horizontal nystagmus with a supposed pendular waveform (1461).
