atinada elección del antidepresivo no es una empresa fácil, sino una
pauta condicionada por variables diversas, sobre todo la forma clínica, la categoría de la enfermedad depresiva, la personalidad, el
estado somático, la situación en la vida, la edad y los antecedentes.
La dosificación pertinente para los cuadros larvados dolorosos o con
otra sintomatología somática precisa ser bastante más ligera de lo
habitual. En cuanto al tratamiento de los enfermos depresivos, creo
que exige como pauta previa el establecimiento de un juicio diagnóstico que englobaría al menos el depresograma, la etiología concurrente o los factores de vulnerabilidad, el tipo de personalidad, la
comorbilidad y los datos biológicos. A partir de estas precisiones
psiquiátricas específicas se desarrolla el seguimiento del enfermo
donde muchas veces es imprescindible la colaboración de un médico general o especializado.
