In patients with unfavorable anatomy prone to collapse, hypoxia and hypercapnia may cause vascular volume to increase thereby further increasing the soft tissue pressures exerted on the upper airway lumen.

This increase in vascular volume may also be caused by other comorbidities such as congestive heart failure, hypertension, and renal disease, for which lying flat may cause a rostral shift in volume.

This rostral shift in volume may be seen in various positions required for surgery, including supine and prone position.
