Insurance policy contract
Literally "accident insurance set to credit cards," obviously, but I am unclear on the precise meaning and there has to be a more elegant term of art for this.

Accident insurance 'provided by' or 'attached to' credit cards
'Set to' would be too literal and doesn't really work in English. 'Set' in katakana is meant to express things made to be a combo but here it means more like 'included' or 'provided' or 'attached to'.
Example sentence:
I've used accident insurance attached to my credit card without any problems.
