¡Hola a todos!
Estoy traduciendo un formulario de 2 páginas que deben rellenar los padres con información sobre el historial médico de sus hijos y entregarlo a la escuela. El nombre del formulario es: "Interval Health History Form".
Me gustaría saber qué significa aquí la palabra "Interval".
Encontré un formulario similar para atletas de Nueva York y lo llamaban "Historial de salud intermedio", pero no estoy muy seguro de esa traducción. En otra traducción de otro estado, omitieron la palabra "Interval" al traducir el nombre del formulario al español.
Pensé en "Historial de Salud del Intervalo" pero no me convence.
