Тук май става въпрос за електроди, макар че от пояснението, което сте дали

се вижда, че „lead” не може да се преведе еднозначно.

intended to record and stimulate electrophysiological activity – това е електрод, с него се отчита електрофизиологичната активност, но с него може и да се стимулира такава активност. Това е приспособлението, което влиза в контакт с живата тъкан (независимо каква е тя). Отвежданията са кабелите/проводниците, които свързват електродите и регистриращото или генериращото ел. импулси устройство.

accurate placement of electrodes and other instruments – това е водач, с чиято помощ електроди или други мед. инструменти, се поставят на желаното място, обикновено вътре в тялото, които без водач не могат да се поставят прецизно.
