I checked the glossary but "selectively" doesn't seem to fit here.

Description of possible metastasis:
"...daneben punktuell schwach anreichernde Herde in der linken Thoraxwand."
Does this have to do with the foci themselves? Which word does "punktuell" actually refer to?

I think the idea is that it is just at particular "points", not as it were joined up - that would be my layman's interpretation.

It's an adverb, of course, so it goes with "schwach anreichernde" - the Herde are increasing/growing in some places.
