My condolences, my father passed away last month. We wouldn't use all capitalised words. 'Altijd en voor altijd' looks awkward but has a poetic ring in Dutch. 'Vader, voor altijd' would be a good translation too. The comma is important because without it the sentence would read someting like 'always a father'.

Frechland I don't know. If your father is from Friesland, another translation might be more appropriate. The peope from Friesland have their own language and they're very proud of it. Frisian is a KudoZ language.

Can you tell us form what city or region he came? Came his mother from the same region? If so, what is her maiden name?

I would translate it this way:
Vader, eeuwig en altijd
or
Eeuwig en altijd vader

('father, eternal and ever' or 'eternal and ever father')

Don't write both eeuwig and vader with a capital, because that would be God, more or less.

there\'s some truth in what you say. The problem is that it is not clear to me whether \'I\' or \'we\' are allowed, otherwise I\'d prefer:

Je blijft altijd mijn vader (\'you\'ll be my father forever\')
or
Je blijft altijd onze vader (\'you\'ll be our father forever\')

A bit further from the source text and a bit less poetical, but this comes more from the heart, I think.
Father, he will always be on our mind. We will never forget him.
Literally translated "tot in der eeuwigheid", until eternity.

My condolences too. I think that if you want to engrave this words in something that is personal and belongs to you, you should use more personal words. Only 'Vader' sounds a little distant in my opinion. Using 'mijn' (=my) gives a personal and warmer feeling.
About the region, you better check of Friesland is the right one. There are lots of windmills in Noord-Holland too, and in West-Friesland (this is not a part of Friesland but of Noord-Holland) and the Zaanstreek (also Noord-Holland), all with their own way of speaking dutch.

This would be Frisian for "father, for always'
