Have an overhang in middle of island countertop.

...to act as a breakfast bar

In other words, the countertop partially extends beyond the pedestal or cabinet below, with enough room for chairs or stools so that people can sit there

Standalone (separated) cabinet

What is the question here? You are unfamiliar with the term "island" or "countertop" or what?
Do you need it translated into another lng?

Anyway, island is a standalone cabinet, that might or might not also have a sink or an oven. This cabinet has a countertop that can be made of various materials such as natural stones( granite, marble, sedimentary rock) or laminated surfaces at the cheaper end ( made of pressed layers of plastic and paper)

I interpreted the sentence as a wish from the client, expressing their idea to the professional. If it's the other way around, the professional can say: What do you think of extending the island...

I don't know what you mean by "come out in the middle of the island". If you mean there will be an attached table at a lower level, and that's why you said "come out in the middle", I'd suggest saying:
Maybe attach a table to the island (with same countertop material or not), with chairs on each side for a/the breakfast area
