2. Cyclodestructive procedures

The ciliary body is a small gland running around the circumference of the eye behind the iris. If filtration surgery fails, an alternative is to destroy a part of the gland to decrease the amount of fluid produced in the eye. Formerly, freezing the gland was the only way to do this. At the present time, various lasers are used. Complications include decreased vision in a significant proportion of patients. The newest procedure, endoscopic cyclophotocoagulation, involves inserting a laser directly into the eye so as to treat only the ciliary body and not surrounding structures. Complications include cataract formation. Many surgeons regard these procedures as a last resort.
