From the emergency admission record of an 82-year old patient with a history of NSTEMI, CABG, IMA, first-degree AVB, and many other abbreviations:

" ... sottoslivellamento di ST in lateral alta, *T difasica-negativa in inferiore*, alterazioni gia' segnalate nell' ultimo tracciato del xxx."

I understand that this is a finding in the inferior leads of the ECG, but will appreciate help with how to phrase the whole thing.

Ambiguous wording I am not sure about the Italian use, but the English term 'biphasic negative' (and corresponding German, FTM) is not clear. A biphasic wave has, by definition, a negative portion, and the distinction between normal and pathological is in most cases whether the negative or the positive portion of the biphasic T-wave comes first. 'Biphasic negative' could then indicate a biphasic T-wave in some inferior leads and a negative (inverted) T-wave in others, or biphasic T-waves with a terminal negative portion (pathological variant), or biphasic T-waves with a negative beginning (usually less significant).

Hi Joseph,

As usual, Anne is very thorough in her answer. I completely agree with her explanation. Biphasic T waves can of two types: those with terminal negativity or with terminal positivity ( also called 'T wave with up-down deflection' and down-up deflection, respectively). T wave terminal negativity is more significant and specific for coronary artery disease. So, with a little bit of guessing, I believe that they are referring to a biphasic T wave with terminal negativity or up-down deflection. But since the source text is indeed ambiguous/imprecise, you may want to make the translation a bit ambiguous as well and say biphasic negative T wave, and noone can accuse you of over-translation or incorrect interpretation ( of the poor source text, but nevertheless).
