Auto de notícia, I found it on IATE and two other reliable sources for European PT.

There’s an article on quora.com that explains the difference between the 2 terms I’m suggesting.

In the context, "official report" is more fitting than "police report", as it is not necessarily from the police. So I'm using that and "incident report".
