Although more context is necessary for full certainty, the sentence sounds like either the patient is undergoing some sort of therapy - "under therapeutic effect" - or the exam may have a therapeutic effect at a certain intensity that must be avoided - "below therapeutic effect".

Carried out under the effect of medication
The only thing I can think of is that the patient was taking medication such as a beta blocker which prevented them reaching target heart rate during the test, hence sub maximal heart rate was achieved.
