Hernia repair discharge note
From a progress and discharge note following hernia repair surgery with mesh, male patient aged 53 with diabetes and obesity. The document is from Mexico.
Diverticular hernial sac? As far as I can tell, all three words seem to refer pretty much to the same thing.
A Google search for "saco herniario diverticular" (in quotation marks) gets no hits, which suggests to me it may be redundant or poorly drafted. Is this just referring to the size of the hernia itself?
Into U.S. English.

diverticular sac

Comment: "Translated as "diverticular hernial sac", as per Stephen's comment; consulted a friend who's a medical doctor in Mexico and she said it was unusually worded but "diverticular" probably referred to the shape of the sac. Many thanks to all who responded."

diverticulum in hernia sac
I think they're not the same thing. Maybe "diverticular" refers to the diverticula that may be present in the sac. I leave some references here. Hope it helps.
