Consequently, it can be considered:

a) That her good conduct is recognized and that she meets the ethics and honour conditions provided by Annex VII. 1. d), regarding article 50.1 of Directive 2005/36/EC, according to its current wording after Directive 2013/55/EU, 20 November;

b) That there is no record that, during her period as registrant, any report, disciplinary proceedings or punitive measures have been filed against her as a nursing professional, meeting all the requirements of the aforementioned Annex VII. 1. d) and g) of Directive 2005/36/EC, according to its current wording after Directive 2013/55/EU, 20 November;

c) That she is not known to be in a situation of physical or psychic illness that impedes her professional practice, as it is stated in Annex VII. 1. e) of said Directive, according to its current wording after Directive 2013/55/EU, 20 November.

This certificate is signed with the approval of the President of the Nursing Board in Ciudad on 19 April 2014
