Source text - French Début de l'affection (évolution, traitements antérieurs éventuels) Patient agé de 50 ans, adressé par le Dr Dominique HONNORAT.
Octobre 2004 : THYROIDECTOMIE TOTALE CURAGE GANGLIONNAIRE suivi d'une totalisation isotopique à 100 mci d'iode 131 le 20.12.04. Il s'agissait d'un carcinome papillaire thyroïdien d'une lésion pT4N1 un ganglion du curage jugulo-carotidien gauche envahi avec effraction capsulaire - d'autres ganglions jugulo-carotidien et spinal gauches avaient été prélevés dont deux étaient envahis avec effraction capsulaire - lobe thyroïdien droit normal - lobe thyroïdien gauche : carcinome papillaire de 3mm de diamètre polaire supérieure gauche. L'exérèse est complète.
Le patient a ensuite trois récidives ganglionnaires cervicales gauches traitées à chaque fois par chirurgie (Dr VAN VOX SOM à la Clin. Jeanne d'Arc en avril 2005, septembre 2006, septembre 2007) suivi d'une 2ème dose d'iode 131 de 100 mcu en Aout 2055 et en Novembre 2006.
Absence de fixation sur la dernière scintigraphie corporelle totale.
Parallèlement sous défrenation, la thyroglobuline initialement à 280 est passée à 33 en 2005, puis 10 µg/l en 2006.
Sous traitement freinateur de LEVOTHYROX 150, la thyroglobuline à 10.5 en mars 2007, découverte à la palpation et en échographie d'une nouvelle adénopathie cervicale gauche. Une TEP TDM au 18 FDG est pratiquée en aout 2007 et montre une hyperfixation au niveau jugulo-carotidne gauche sans autre fixation.
Le patient est donc à nouveau opéré par le Dr VAN VOX SOM le 25.09.09 : il existait en fait deux adénopathies macroscopiques, trois micro-adénopathies à distance. Sculpture de l'adventice carotidienne puis de la paroi jugulaire. La lésion était adhérente au nerf vague et au nerf sympathique cervical.
Il s'agit d'un adénocarcinome papillaire peu différencié thyroglobuline .
Le dossier est discuté en RCP le 25.09.07. Il est décidé une radiothérapie sur la région cervicale.
A noter que le dernier dosage de Thyroglobuline le 9,11.07 montre un taux à l0.8 µg/l.

October 2004: TOTAL THYROIDECTOMY LYMPH NODE CLEARANCE followed by radioiodine remnant ablation with 100 mCi of iodine-131 on Dec. 20, 2004. It was a papillary thyroid carcinoma, stage pT4-N1 – a lymph node from the clearance of the left carotid-jugular chain invaded with capsular extension – other left spinal and carotid-jugular chain had been sampled of which two were invaded with capsular extension – right thyroid lobe normal – left thyroid lobe: 3-mm papillary carcinoma in left upper pole. Total ablation.
The patient then had three left cervical lymph node recurrences treated each time surgically (Dr. VAN VOX SOM at Clinic Jeanne d'Arc in April 2005, September 2006, September 2007) followed by a 2nd iodine-131 dose of 100 mcu in August 2005 and in November 2006.
Absence of fixation on the last whole-body scintigraphy.
In parallel, unmedicated, thyroglobulin levels initially at 280 went down to 33 in 2005, then 10 µg/l in 2006.
During suppressive therapy with LEVOTHYROX 150, thyroglobulin at 10.5 in March 2007, but in June 2007, detection upon palpation and ultrasound of a new cervical adenopathy on the left. An 18FDG PET/CT was performed in August 2007 showing a hyperfixation on the left carotid-jugular chain without any other fixation.
The patient was therefore operated on again by Dr. VAN VOX SOM on Sept. 25, 2009: there were in fact two macroscopic lymph node enlargements, three micro-lymph node enlargements nearby. The carotid adventitia was sculptured then the jugular wall. The lesion adhered to the vagus nerve and the cervical sympathetic nerve.
It was a thyroglobulin positive, poorly differentiated papillary adenocarcinoma.
This case was discussed in a multidisciplinary meeting on Sept. 25, 2007. A decision was made in favor of radiation therapy on the cervical region.
It should be noted that the last level of thyroglobulin on Nov. 9, 2007 shows a level of l0.8 µg/l.

Source text - French Or, pour la fabrication de ces panneaux, 500 000 tonnes de résines sont annuellement utilisées rien qu’en France et l’ensemble de ces résines, du type Urée-Formol (UF) et Mélamine-Urée-Formol (MUF), est composé de produits contenant des proportions très importantes de formaldéhyde.
La réaction chimique de durcissement de ces résines est une polycondensation et, de ce fait, il est matériellement impossible que 100% du formol participe à la réaction. Ainsi, malgré les énormes efforts réalisés par la profession et les avancées de la technologie mises en œuvre par les fournisseurs, il existe toujours un pourcentage de formol libre dans le panneau.
Par ailleurs, comme toute réaction de polycondensation, les résines UF et MUF sont sensibles à l’hydrolyse. Ceci se traduit dans les faits par la présence d’émanations de formol lors de l’utilisation des panneaux, notamment en milieu domestique du fait de leur emploi, par exemple, pour la fabrication de meubles.
Bien que extrêmement faible, compte tenu du classement du produit, ces dégagements sont considérés comme un danger potentiel et il importe de trouver des produits et des procédés permettant leur diminution sensible, voire leur disparition.
Conscients du problème de santé publique, les producteurs ont fait évoluer les panneaux présents sur le marché.
Translation - English However, in order to manufacture these panels, 500,000 tons of resins are used annually in France alone and all of these Urea-Formaldehyde (UF) and Melamine-Urea-Formaldehyde (MUF) type resins are composed of products containing very high proportions of formaldehyde.
The chemical curing reaction of these resins is a polycondensation and, for that reason, it is materially impossible for 100% of the formaldehyde to participate in the reaction. As a result, despite the major efforts made by the industry and technological progress made by suppliers, there is still a percentage of free formaldehyde in the panels.
Moreover, as in any polycondensation reaction, the UF- and MUF-resins are sensitive to hydrolysis. This is demonstrated by the presence of formaldehyde emissions when the panels are used, in particular in a household environment when used, for example, to manufacture furniture.
Although extremely low, given the classification of the product, these emissions are considered a potential danger and therefore products and processes need to be found to substantially lower or even eliminate them.
Aware of the public health risk, producers have improved the panels that are currently available on the market.

Il me disait également que chaque maison était unique et possédait son secret et que le jour où je découvrirais celui de ma maison, je me mettrais à l'aimer comme ma meilleure amie.
Un jour que je peinais à mon bureau, occupée à apprendre une poésie (c'était pour moi une rude épreuve car je n'arrivais jamais à me concentrer), le capuchon du stylo que je mâchonnais a sauté derrière mes pots à crayons. Je tâtonnais pour le récupérer quand j'ai senti du bout des doigts un petit tiroir que je n'avais jamais vu. Je l'ai ouvert : il contenait une clé montée sur une cordelette en laine noire, avec une lettre qui disait ceci:

J'ai regardé la clé avec attention. Il y avait une inscription dessus, "Villa Volubilis". Exactement la même que dans le vestibule de la maison. Je ne savais pas ce que je devais chercher, mais cela ne faisait aucun doute, le jeu de piste commençait dans le vestibule.

La fureur des combats de la journée a laissé place au silence apaisant de la nuit. L'air est cristallin. Aucun nuage ne filtre l'éclat blafard de la pleine lune. On y voit presque comme en plein jour.
Entre les tranchées de chaque camp s'étend un no man's land de boue (figée) piqueté de barbelés, comme autant de cicatrices.
L'endroit semble désert, mais est en réalité peuplé de cadavres et d'agonisants disséminés autour et à l'intérieur des nombreux trous d'obus témoignant de la violence de l’affrontement.
Une silhouette plane d'un cratère à l'autre.
2.EXTERIEUR – CHAMP DE BATAILLE(Sur une butte de terre) – NUIT
La silhouette est immobile. Du sommet d'une petite butte elle domine le champ de bataille.
Il s'agit d'un jeune homme aux traits fins et glabres, presque enfantin. Dans son uniforme bleu nocturne il embrasse la ligne d'horizon d'un regard circulaire, les sens aux aguets.
Il semble humer l'air frais de la nuit dans l'attente d'une réponse. C’est l’INCONNU.
Un gémissement de douleur attire son attention.
L'Inconnu s'élance d'un bond surnaturel.
3.EXTERIEUR – CRATERE #1 – NUIT
Dans son uniforme vert de gris le SOLDAT ALLEMAND gît au fond de son trou, gémissant de douleur. Aucun insigne de grade sur sa tenue de combattant. Il s'agit d'un simple soldat.
Dès qu'il l'aperçoit, il s'adresse à l'Inconnu qui le surplombe.
D'un geste de la main, l'index sur ses lèvres, l'Inconnu lui intime de se taire. Il observe le Soldat Allemand blessé, le jauge.
Il prend le temps de la décision avant d’agir.
Après un temps, il lui jette une gourde pleine et, d'un bond trop rapide pour l'œil, disparaît.
4.EXTERIEUR – CHAMP DE BATAILLE – NUIT
Deux hommes, DETROUSSEUR #1 et DETROUSSEUR #2, rampent au milieu des débris et des corps du champ de bataille. Leurs visages sont enduits de suie, leurs mines sinistres.
Ils sont vêtus d'uniformes Français dépenaillés, décorés çà et là d'oripeaux évoquant les « Apaches ».
Ils portent des poinçons de tranchée de leur fabrication, et progressent tête nue, sans le casque Adrian.
Ils se font le plus discrets possible, se cachant en permanence des regards/veilleurs des deux camps.
5.EXTERIEUR – CRATERE #2 – NUIT
Un SOLDAT FRANÇAIS lutte depuis des heures au fond d'un trou. La jambe coincée sous un espar, il repousse inlassablement au moyen d’une pelle la boue qui coule et menace de l'ensevelir.
Sans un mot l'Inconnu s'approche et soulève la poutre d'une main.
Il la maintient sans aucun effort apparent et, de sa main libre, aide le Soldat Français à s'extraire de son piège.

1.EXTERIOR – BATTLEFIELD – NIGHT
The day’s raging battles have been replaced by the night’s soothing silence. The air is crystal-clear. Not a single cloud hinders the pale shine of the full moon. We can see almost as in broad daylight.
Between the trenches of each camp, a no man’s land of mud stretches out, (frozen) dotted with barbed wire, like scars.
The place feels deserted, but is in fact populated with cadavers and soldiers agonizing scattered in and around the countless shell holes, testifying to the violence of the confrontations.
A silhouette looms over one crater after another.
2.EXTERIOR – BATTLEFIELD (On a mound of earth) – NIGHT
The silhouette is motionless. From the top of a small mound, it overlooks the battlefield.
It’s a young man with fine, smooth features, almost childlike. In his dark blue uniform, he embraces the horizon with a circular gaze, all his senses on the lookout.
He seems to sniff the night’s cool breeze, awaiting an answer. He is the UNKNOWN.
A moan of pain draws his attention.
The Unknown rushes forward in an eerie, ghostlike leap.
3.EXTERIOR – CRATER #1 – NIGHT
In his verdigris uniform, the GERMAN SOLDIER lies at the bottom of his hole, moaning in pain. There is no rank insignia on his battle uniform. He is a private.
As soon as the German soldier sees the Unknown looking down at him, the soldier begins to speak.
With a sign of his hand, his forefinger on his lips, the Unknown instructs him to keep quiet. He watches the wounded German Soldier, sizing him up.
He takes his time, making up his mind, before acting.
After a while, he tosses the soldier a canteen full of water and then quickly leaps, disappearing in the wink of an eye.
4.EXTERIOR – BATTLEFIELD – NIGHT
Two men, ROBBER #1 and ROBBER #2, crawl amongst wreckage and bodies on the battlefield. Their faces are covered with soot, their expressions ominous.
They are dressed in dilapidated French uniforms, adorned here and there with trimmings reminiscent of Apaches.
They carry trench daggers they have made themselves, and move forward bareheaded, without M15 Adrian helmets.
They are as inconspicuous as possible, constantly ducking out of sight from both camps.
5.EXTERIOR – CRATER #2 – NIGHT
A FRENCH SOLDIER has been struggling for hours at the bottom of a hole. With his leg stuck under a girder, he tirelessly shovels back the mud that is sliding down and threatening to bury him alive.
Without a word, the Unknown comes closer and raises the beam with one hand.
He holds it up without any noticeable effort and, with his free hand, helps the French Soldier out of the trap.
