French to English Law/Patents Law (general) document serving

UN EXEMPLAIRE DE L’ACTE DOIT ETRE RETOURNÉ AVEC L'ATTESTATION DE SIGNIFICATION OU DE NOTIFICATION (Article 4, paragraphe 5 du règlement)

ONE COPY OF THE DOCUMENT MUST BE RETURNED WITH THE DECLARATION of DOCUMENTS TO BE SERVED OR NOTICE TO BE GIVEN (Article 4(5) of the Regulation)

Again, it is unclear which regulation is referred to.


Although I already looked up 'attestation' in ProZ, and at Eurodicautom (not to forget my dictionaries, I ended up being more confused as this appears to have many meanings. My common sense tells me this is simply a 'signed receipt' rather than a certificate, but cannot be sure.

This is the context:
DEMANDE DE SIGNIFICATION OU DE NOTIFICATION D’ACTES
REQUEST FOR DOCUMENTS TO BE SERVED OR NOTICE TO BE GIVEN

I also have the sneaky feeling that I am reinventing the wheel. There must be standard forms for this sort of thing. In addition, I tried finding the European regulations referred to here online.
