For answers to questions about this research study, study procedures, or to report a research-related injury, contact the study doctor: at on .
I am assuming that 'at' will be followed by a phone number. Such phone numbers are usually available 24/7 for all study participants.
Would 'on' denote a day of the week or am I missing something?

{name of doctor} at {name of establishment} on {phone number}
In UK English at least, 'on' is more likely to refer to a phone number than 'at'
I think, therefore, that the balance of probability would suggest that the 'at' refers to a place (the clinic/hospital/dept. etc.), and the 'on' refers to the phone number

: would be my choice, too. As these topics are NOT emergency related, they put somebody on the phone to answer these questions only on Thursdays.

: My first thought was telephone number: "Dr X at ABC University Hospital on 123-4567" but you're right to say that the day of the week is also possible

It could be a date but I think it's also possible that doctors are assigned to provide information on a specific topic of the study.
