A figura de linguagem aí é com "war of attrition" (guerra de atrito). Uma guerra de atrito é um conflito arrastado, onde os dois lados vão esvaindo seus recursos (militares, econômicos, humanos, etc) e ganha quem ficar sem recursos por último. Um exemplo foi o front ocidental da Primeira Guerra Mundial. O autor está insinuando que a luta contra a AIDS é uma guerra de atrito que está se exacerbando.

Acho que uma tradução literal não ficaria bem... o ideal seria outra metáfora ou frase que se destacasse no texto...

Existem várias outras possibilidades, e como se trata de Pt-Pt, saberás escolher melhor que um não-lusitano como eu.

A definição física de atrito é a de uma força que se opõe a outra que seja aplicada; no contexto apresentado, penso que o autor se refere a toda uma problemática que cada vez mais impede a eficácia do socorro às populações afectadas pelo HIV (diversos programas nacionais, burocracia, interesses económicos, etc.). A sugestão de 'atrito institucional' parece estar de acordo com o tom que referiu.
