What I understand from just looking at this is that the patient is fully awake for the operation and therefore watching anxiously as the camera goes along their artery.

First of all, "on" is probably best translated by "you" here, as the person being operated is describing what "one" would feel ("you" is more informal for "one").
There is an idea of "by chance/accident" (passivity) here, imho. The person being operated on is acting like it he just happens to be there lying on an operating table admist all the surgical procedures.
