I know it is possible to remove a prostate gland 'bit by bit' - or break it down and have bits of it pass out in the urine, but I am not sure that is what is intended here, with this 'recoupe' (presumably some sort of re-excision)? Any light thrown on this would be very helpful!

Note that "recoupe" does not necessarily mean "cut again".
It can also be used in the sense of "trimming", "tidying up" or removing extra tissue to enlarge the resection margins (see "recoupes saines" = negative resection margins).
Note also that "resection" means removal (excision).

No. Prostate resection = removal of the prostate = prostatectomy. That doesn't fit here as a step in the actual surgical technique. The prostate has not yet been removed at this stage - this is only referring to detaching it from the urethra. Then the surgeon goes on to dissect the prostate from the tissue lying behind.

"Pièce" is usually "pièce anatomique" or "pièce opératoire" = specimen. I wonder if it doesn't mean that the prostate itself was sectioned/incised.

The bit-by-bit operation you're referring to is a transurethral resection of the prostate - a re-bore more or less (ouch). The operation in your text is an open prostatectomy. The part posing a problem is about sectioning the urethra where it joins the prostate.

incision

Here, an "incision of the prostate" was justified in order to do the surgical resection. It was expressed this way to justify making an incision into the organ, which ordinarily might be avoided to maintain the integrity of the specimen and avoid any crossing of tumor extensions. Here the gross margins ("marges macroscopiques") were visibly OK.

I base the above on the excellent reference Dr. SJLD put into the discussion above and on my experience with numerous prostatectomy specimens in the time of my surgical pathology practice.
